The George Conti, Jr. Memorial Scholarship Application
TO THE APPLICANT: The Applicant must be a graduating High School Senior. Please
complete this application with as much information as possible. If the space provided for any
question is inadequate, feel free to attach additional information. All applications should be
submitted with at least two letters of recommendation, more if possible, with at least one
recommendation related to the candidate’s soccer background. A current picture is required and
should be attached to this application in the box below.
