Please read the attached General Principles and Implementing Guidelines. Then using the attached grid (Appendix
I.7.A) as your guide, list your outputs. (See examples below.) This application form must be accompanied by a portfolio containing supporting materials, e.g., photos, reviews, citations, etc. Do not fill in the blanks for points in this application form. These are for the evaluators to fill. Use extra pages if necessary.
There is a separate grid for each arts discipline and for arts scholarship. If you have outputs in more than 1 art discipline, you must fill out separate forms. Points earned for all arts will be totalled.
UP must appear as the artist’s base/address for the works to be credited.
Please use the format followed in the sample entries below.
Sample entries:

An applicant who earns at least 70 points is eligible for UP Artist 1.
An applicant who earns at least 95 points is eligible for UP Artist 2 , provided some of these points are earned through the recognition of the quality of these artistic outputs in the form of at least one award or distinction such as those described in Appendix I.7.B

An applicant who earns at least 120 points is eligible for UP Artist 3.
In addition, s/he must have attained recognition of outstanding achievement through at least one lifetime achievement award or distinction such as those described in Appendix I.7.C.

Large-Scale Projects (built)
Evaluation will take into consideration the scale and complexity of the project. Design and Building Systems include only those with artistic components (i.e., lighting design, acoustic design, and the like).
Large-Scale Projects include large institutional, commercial, and multi-unit residential projects: buildings such as the Cultural Center of the Philippines, Quezon Hall, the Greenbelt Complex in Makati, Manila City Hall, large condominium projects, monumental structures, airport, large bridges, and other structures of increasing complexity.
Also included are major restorations, large master and site development plans, expansive parks and open spaces.
