GIRLS BASKETBALL TRYOUTS
RSMS will have basketball tryouts on Friday May 13th for students in 6th and
7th grades. Please make sure you wear proper clothing (gym shoes, shorts,
socks, and t-shirt.)
YOU MUST HAVE A RIDE TO PICK YOU UP ON TIME!
Time: 3:20 to 5:00 at Rock Springs Middle.
I give my child permission to try out for the RSMS basketball Team and waive
and release RSMS and the coaching staff from any claims of personal injury that
may result from an accident during tryouts.

 Have this form completely filled out and signed by your teachers and parents.
 Have the completed TSSAA physical form and notarized insurance form.
 A copy of your last report card.

