All female athletes interested in trying out
for the CMS 7th or 8th Grade Girls Basketball
Teams should obtain the Athletic Forms Check
List from the school office. The documents in
this packet must be turned in to the office BEFORE the first day of tryouts (by Monday, August 31). These documents include:
 Physical
 Extracurricular Code of Conduct
 Parent Permission Slip
 Insurance Waiver
8th Grade Tryouts
Monday, August 31—6:30-8:00 a.m.
Tuesday, Sept. 1 —6:30-8:00 a.m..
7th Grade Tryouts
Monday, August 31—6:30-8:00 a.m.
Tuesday, Sept. 1 —6:30-8:00 a.m..
