insurance company may require a referral to a doctor. Also please check with the doctor’s
office, so your family insurance is accepted at that particular office.
(**) After your child is seen by a physician for an injury/condition, a medical clearance
note must be attained from the treating physician and handed to the high school athletic
trainer, in order to allow the student/athlete full clearance for return to CHS sport/gym
class.
