FINANCIAL AID SATISFACTORY ACADEMIC PROGRESS
COMMITTEE APPEAL FORM
Federal regulations require students to maintain “satisfactory academic progress” (SAP) in three areas—
(1) cumulative GPA, (2) percent of hours earned, and (3) maximum time limit—to be eligible for financial aid. It is
the student’s responsibility to stay informed of the college’s SAP standards and to monitor their own progress. As a
courtesy the Financial Aid Office sends notification to students at the end of each semester if a student is out of
compliance with Federal and State satisfactory progress standards. Those students who are required to appeal are
normally given at least one opportunity (with stipulations) to work on bringing their academic standing back to
satisfactory standards.
When a student fails to meet all of the stipulations of their prior appeal(s), they may be required to submit an
appeal to the Financial Aid Appeals Committee. This form is for that purpose.

Attach a typed (signed and dated) personal statement which includes the following:

Describe the unusual circumstances that prevented you from meeting the Standards of Satisfactory Academic
Progress (SAP).
State why you believe it is possible for you to improve upon your past academic performance.
Explain the corrective action you have taken and describe your plan for success.
If you exceeded the maximum attempted hours (150% rule), you must explain why you have
accumulated attempted hours beyond that required for your certificate/diploma/degree.
Attach documents that verify your statement. All documentation must relate to the specific period during which
the student’s academic performance was affected and apply to the circumstance(s) described on the statement.
Documentation must be included in order for the appeal to be reviewed.
6. You are required to submit a Degree Audit (Graduation Checklist) from your Academic Advisor
listing the remaining requirements for your degree program. Your advisor must write in the projected
completion date. Both you and your advisor must sign and date the form.
Examples of acceptable documentation includes but isn’t limited to:







Birth/death certificates, obituaries, funeral programs of immediate family members (i.e. parents, grandparents,
spouses, children, brothers, sisters). Documents must establish the relationship to student.
Medical records and/or signed and dated doctor’s statements on official letterhead that confirm illness and
length of recuperation.
Official court documents or legal documents on official letterhead.
Signed & dated statements from counselors, clergy or social workers on official letterhead.
Signed & dated statement from work supervisor on company letterhead.
Personal statements from family members or friends must be signed, dated and notarized.

Acknowledgements:

I understand that the committee will not accept any Financial Aid Satisfactory Appeal Form that is
incomplete or lacks documentation. I am, therefore, submitting my completed form and documentation.
Once a decision has been made, I will be notified of the outcome within five business days.
I understand that if my appeal is approved, I am expected to meet the following stipulations:
1. I must maintain a GPA of at least 2.0, during my probationary period; and maintain or improve
my overall cumulative GPA to at least to 2.0.
2. I must not withdraw or drop from any classes for the duration of my education at FlorenceDarlington Technical College.
3. I must enroll in only the number of hours granted by the committee. If I choose to enroll in
additional hours, I will be responsible for full payment. (Financial Aid can only be applied to
courses contained within my major.)
4. I cannot change my major without speaking with my academic advisor and the Financial Aid
Director.
5. I must not take courses that are not part of my program’s curriculum requirements. Doing so
could result in me having to pay for the courses that are not part of my academic program.
6. Any other stipulations that the committee may deem necessary.
I understand that if I do not meet all of the terms of my appeal, my appeal will be cancelled and I may be
responsible for repayment of all or part of any aid awarded to me.

withholding or falsifying any requested information may result in rejection of my application and/or expulsion
from Florence-Darlington Technical College. I understand that if my appeal is denied, I am responsible for the
full cost of any tuition, fees, books, or other debts owed to the College. I also understand that if my appeal
application is submitted without supporting documentation, it will not be accepted by the Financial Aid Office
and will be returned to me.
