Display Screen Equipment Assessment Form for Associate Lecturers This self assessment form is intended for use by Associate Lecturers. The assessment should be made by the DSE User at their workstation. The Assessment should be reviewed whenever there is a change to the workstation or equipment. Associate Lecturers have a responsibility to ensure that they take breaks and highlight any difficulties they are experiencing. Unresolved problems should be referred to the Line Manager. Actions Section 3 Workstation and furniture Yes No Is the equipment and furniture suitable? Is the desk large enough for all equipment and work? Does the desk have a non-reflective surface? Is the height of the desk suitable? Is the desk tidy? Can adjustments be made to desk height? Is there adequate space under desk to allow for change of posture? Are there any other problems associated with the workstation? Associate Lecturers are expected to provide a suitable workstation and furniture and have a responsibility to keep their desk area tidy. Can the height, the tilt and the angle of the chair back be adjusted? Does the User know how to adjust it? Do chair arms restrict movement? Is the chair in good repair? Can the worker sit with feet flat on floor when chair is correctly adjusted? If feet do not reach the floor is there a footstool? Associate Lecturers are expected to provide and use a suitable adjustable office chair. Individuals should adjust their chair for their personal comfort. Actions Section 5 Display Screen Equipment Is the screen readable and the characters stable? Are the brightness and contrast controls well set? If alternative colours for background and text are used are they suitable? Is the user keying properly with hands and wrists level and a soft touch? Can the keyboard be tilted? Is the keyboard well positioned? Is the mouse appropriately positioned? Is there a mouse mat? (not necessarily required with optical mouse) Is the mouse used correctly with hand and wrist level? Is the screen clean? Is the height of the screen satisfactory for the operator? Is there enough space to rest the hands in front of the keyboard? Is computing work mainly on a laptop?(see additional advice on laptop set up) If you use a document holder is it positioned and adjusted correctly? Associate Lecturers are expected to provide suitable DSE equipment which should be positioned to suit the individual’s needs. The screen should be at eye level or slightly lower. Are there suitable adjustable blinds? Is there any obvious distracting noise such as a printer, which could lead to discomfort? Is the screen well positioned to avoid reflections on the screen? Glare should be managed by moving or tilting screens and adjusting blinds. Actions Section 7 The employee Has the employee access to relevant information? Does the person have any IT training needs? Does the person know how to obtain an eye test? Are there any medical symptoms suggestive of visual or postural problems? If you have any issues as a result of completing this form – please discuss them with your Line Manager. This form should be sent to your Line Manager.
