This form is to be completed by the supervisor for any submission made by a candidate and is
required by the Occupational Psychology Qualifications Board. The supervisor should describe the
candidate’s performance in detail so as to provide a rounded view of his/her strengths and
weaknesses and, in addition, rate the candidate’s performance in each section as ‘competence not
yet demonstrated’ or ‘competence demonstrated’.
In making the rating for each section account should be taken of the comments in the various
sections. The following descriptions of the categories should be used to make the ratings:

This may reflect:

some degree of concern over aspects of the candidate’s performance
failure to meet initial goals
minor difficulties in several areas or a major concern (not amounting to
unprofessional or unethical conduct).

Competence
demonstrated:
The reasons for the rating should be obvious from the written comments in
the section.
This is at least as good as the general level of performance expected of the
competent psychologist. As a rule of thumb assessors will consider whether
the candidate could work un-supervised with one of their clients.
It reflects a good match between performance and the initial objectives.
(Where initial goals are not met this is due to unforeseen circumstances or
revision of the goals during their training).
The candidate will have undertaken a reasonable workload and
carried out the work competently
When rating the Process Skills and Consultancy Cycle Skills, note that these must be
demonstrated in each of the Breadth and Depth entries respectively, and therefore competence will
not be demonstrated to the expected level until the required number of entries have been
submitted. Unless AEC has been awarded, no Depth entries will be submitted in the first annual

submission, and hence competence in the Consultancy Cycle Skills will not be demonstrated at
this time.
