INSTRUCTIONS: (PLEASE READ CAREFULLY)
1.
Examination Admission Form and Fee should reach the university on or before the due date of
admission, failing which fee due on the date the form is received, will be charged.
2.
Examination Admission Form and Fee for each term must be submitted separately. No student will
be allowed to appear on a single Admission Form for more than one examination.
3.
Relevant DMC’s of the preceding terms must be attached with the Admission Form.
4.
University Registration Number must be mentioned in the Admission Form.
5.
All students are required to attach three copies of passport size photographs and one copy of
National Identity Card/Domicile Certificate duly attested by the Director/Teacher Incharge
concerned.
6.
Incomplete/unsigned Forms will not be entertained and shall be returned at the cost/risk of the
student.
7.
Admission Fee remitted through Money Order/cheque will not be accepted.
8.
A student who fails to qualify any term in two consecutive chances (availed/un-availed) shall not be
promoted to the next term and shall be required to repeat that part as a whole.
