Appointments
To book an appointment please contact the surgery between 8.30am and 6.30pm
All consultations and clinics are by appointment only –
If you cannot keep your appointment please advise the surgery as soon as possible so that we can
offer the appointment to someone else.
If you feel you cannot wait until the next available routine appointment, please advise the
Receptionist and you will be offered an urgent appointment.
Home Visits
If you are too ill to attend the surgery and you require a home visit please request this before
10.30am by telephoning one of the surgery telephone numbers.
Please give the Receptionist as much information as you can as this will help the Doctor to
prioritise his calls.
Telephone Advice
You may telephone the surgery between 12.00 noon and 1.00pm for a telephone consultation
with the Doctor or Nurse.
Repeat Prescriptions
If you are on regular medication you will probably not need to see the Doctor to obtain a repeat
prescription. Please tick the items you require on the computer slip which is attached to the right
hand side of your prescription and place it in the box at Reception.
Let the Reception staff know if there is any medication on the list which you are no longer
taking.
Please allow 48 hours for repeat prescriptions. If your request is urgent please advise
Reception who will make every effort to have your prescription ready the same day.
Please do not telephone for repeat prescriptions. These should be requested in writing by
calling into Reception or by posting a request through the letterbox if the surgery is closed.
If you would like your prescription posted, please enclose an SAE.
Our local Pharmacies offer a prescription collection service. Please let Reception know if you
would prefer to collect your prescription from:
Reception Moors Pharmacy,
Farm Road,
West Moors
Boots the Chemist,
Tricketts Cross (next to Sainsburys)
To help us minimize mistakes, may we ask that you collect your prescription from the same
place each time.
If you live more than one mile from a Pharmacy, we are a Dispensing Practice and will
supply your medication(s) directly to you.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought
it reasonable to charge for those who can afford to pay for their medicines. Prescription
prepayment certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
From 1 April 2009, the charges are:



Prescription (per item): £7.20
12-month prepayment certificate (PPC): £104.00
3-month PPC: £28.25
If you will have to pay for four or more prescription items in three months, or more than 14 items
in 12 months, you may find it cheaper to buy a PPC. The charge for a single prescription item is
£7.20, whereas a three month PPC will cost you £28.25 and a 12 month PPC £104.00.

There is further information about prescription exemptions and fees on the NHS website
(www.nhs.uk/NHSEngland/Healthcosts/Pages/Prescriptioncosts.aspx)
Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed.
Any problems please telephone the surgery.

hospital transport.
Please telephone for test results after 3pm.

medical certificates, countersigning passport forms and photos, some immunisations, completion
of private health insurance forms, certain private letters, various medical examinations. Please
ask Reception staff for details.
Registration
New Patient Registration

to complete a health questionnaire and are offered a consultation with the doctor.
Medical treatment is available from the date of registration. Please contact reception for further
information.

Comments & Complaints
We make every effort to give the best service possible to everyone who attends our practice.

genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly,
and as amicably, as possible.
To pursue a complaint please contact the practice manager and she will deal with your concerns
appropriately. Further written information is available on the complaints procedure from

We are continually striving to improve our service. Any helpful suggestions would be much
appreciated and a suggestion box is located in the waiting area.
