When We Are Cosed

Out of Hours: For emergencies outside normal hours, a recorded message will give you details of the number to contact to request emergency treatment. Please ensure that you have a pen and paper handy to take down the information given. Currently this number is 111. Under the GMS Contract, out of hours emergency calls will be dealt with centrally and from Warwick Hospital and these arrangements are made by the PCT. Call 999 in an emergency. Chest pains and / or shortness of breath constitute an emergency.

CHANGE OF CONACT DETAILS: Please let the surgery know if you change your address and telephone contact details. This can be done by completing a form at reception or by completing the form on-line www.abbeymedicalcentre.co.uk

It is important for the practice to have up-to-date details listed for each patient registered with the practice.

NHS CHECKS:

The NHS health check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia through proactive screening for 40-74 year olds. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of the conditions above or have certain risk factors, is eligible for a FREE NHS health check to assess their risk of the above and will be given support and advice to help them reduce or manage that risk.

New Service to promote health in patients aged 75 years and over

Along with other practices in South Warwickshire, we have recently launched a new service which aims to provide holistic clinical reviews for those aged 75 years and over.

The purpose of the review is to undertake an in-depth assessment of the health status of the person being reviewed, with a view to considering what pro-active measures may be helpful in helping the person maintain good health.

Additionally, for those who are more frail or who have poorer health we have teamed up with Age UK Warwickshire who will undertake a thorough assessment covering aspects such as the ability to look after oneself on a daily basis, transport, financial wellbeing and safety. It is hoped that these comprehensive reviews will complement each other and we will be able to improve the care that we provide to individuals, as well as signposting them onto other services and resources that can assist in living healthily and happily.

The practice will be contacting patients individually throughout the year to offer them the opportunity to participate in this service.

The “over 75s project” is a pilot running until 31 st March 2016. We will be asking for feedback and encouraging comments from those who use the service. There is potential for it to continue if we can show we are having a positive effect on the health and wellbeing of the people who use this service.

Asthma Reviews: If you are unable to attend the surgery for your annual review you can complete a form on the surgery website.

Pneumococcal Vaccinations: All patients 65 years and over should have this vaccination.

Travel Vaccinations: If you require travel vaccinations please could you book your appointment with the nurse at least 6 weeks in advance before your travel date to ensure you are adequately protected.

Warfarin Clinic: Please note the Warfarin Clinic is now on a Thursday morning. www.abbeymedicalcentre.co.uk

PATIENT PARTICIPATION GROUP

Abbey Medical Centre have a Patient Participation Group, who meet once every quarter.

The purpose of this group is to listen to our patients and look at ways to make improvements where possible. This group should not be seen as a sounding board to complain. Complaints should go through the normal process and will NOT be dealt with by the group or at the meetings.

PRESCRIPTIONS

Please allow TWO working days for your repeat prescription to be processed.

Collection should be after 3.00 p.m (see details below).

Unfortunately we are unable to take requests for prescriptions over the telephone, though you may fax your request on (01926) 851746 or visit the website at www.abbeymedicalcentre.co.uk

and follow instructions.

Repeat Prescriptions

If you are on regular medication your doctor may issue you with a repeat prescription. This will be reviewed from time to time. On the right-hand side of the prescription is a tear-off sheet giving details of all the repeat medication you are on. It is important that you use this slip to order further prescriptions.

Tick the items you require and either drop it into the box in reception or post it to us.

Please enclosing a stamped, addressed envelope if you wish us to return it to you.

LOCAL PHARMACIES OFFER A REQUEST AND COLLECTION SERVICE FOR PRESCRIPTIONS AND WE

RECOMMEND THAT YOU USE THIS FREE SERVICE. PLEASE LET US KNOW WHICH PHARMACY YOU

WOULD LIKE TO COLLECT YOUR PRESCRIPTION FROM WHEN ORDERING.

SURGERY NEWS:

Dr Sam Young:

We are happy to announce that Dr Young safely delivered a baby boy born in November 2015.

Congratulations to Dr Young and her family.

Dr Hannah Bakewell:

Dr Bakewell is our new locum GP covering for Dr Young while she is on maternity leave.

Dr Sally Cooper:

It is with regret that we inform you that Dr Sally Cooper left the surgery in November 2015.

After much soul searching she feels that it was time for her to move on to pastures new.

We all at Abbey Medical Centre would like to take this opportunity to wish her well in her future plans.
