Job Description Service Walsall Integrated Recovery Service Job Title Young Person’s & Adults Recovery Transitions Coordinators NM484 Base The Old Clinic, Bradford Street, Walsall Hours 37.5 Salary Range CRI points 23-28 Reports to Young Person’s Team Leader Purpose of Job The post holder will be part of the Walsall Integrated Drug and Alcohol Recovery Services; sitting specifically within the Young Persons element, providing assessment, advice, care planned treatment and support to under 19 year olds experiencing serious and complex substance related problems in the Borough of Walsall You will support individuals through the transition into adult services and also transition in to paid employment and/or relevant training, education or occupation/diversionary activities that can best support there ongoing needs. You will work closely with families, parents & carers and work in a supportive and collaborative partnership approach with other relevant health, social care and criminal justice agencies, including Walsall Manor Hospital and Youth Offending Team. You will contribute to providing high quality services, with a focus on performance and positive outcomes for service users, including a responsibility to understanding and contribute to the service performance and achievement of key targets and outcomes. You will be expected to take an active part in your own development through the CRI supervision and Personal Development Framework The post holder will also deliver group work to children and young people and deliver training and consultation to professionals on referral processes and screening. Key Activities: To deliver a range of treatment interventions to the clients utilising a child and young person centric approach guided by the 10 key policy principles of working with children and young people. This may include both individual and group based client work. To carry out triage/screening assessments where appropriate To work flexibly and in a holistic way to promote harm reduction. To devise, implement and review individual care plans with young people. To appropriately include parents/carers in the planning and delivery of support. To support a caseload of young people, delivering psychosocial interventions alongside basic counselling skills and motivational interviewing approaches. To support individuals through the transition into adult integrated services To support individuals through the transition n to paid employment and/or relevant training, education or occupation/diversionary activities that can best support there ongoing needs. To actively develop relations with partner agencies ensuring robust referral protocols and care pathways are put into place for young people to receive substance misuse interventions and other appropriate support. To offer a flexible service across the Borough of Walsall in partnership with Health, Social Care, Criminal Justice and generic youth services. To participate in the provision of consultancy and training as appropriate to partners and Stakeholders Ensure continual development and improvement of the service to meet the ongoing future needs of new trends, such as Novel Psychoactive Substances etc Ensure that all administration, case recording, safeguarding systems are up to date and are maintained to high standards. General terms of reference: In carrying out the above duties the post holder will: Work flexibly across operational sites as required. Undertake other duties appropriate to the grade of the post Person Specification Essential criteria: Considerable knowledge and experience of working in the substance misuse field. Considerable knowledge and experience of working with children and young people. Knowledge of issues facing young people today. Basic counselling and / or group work experience. Experience of undertaking comprehensive assessments and in implementing and reviewing individual care plans. An ability to maintain accurate and timely records. Understanding of professional accountability in relation to The Children Act 2004 and Every Child Matters 2004 And the ability to: Communicate confidently and effectively, verbally and in writing. Respond flexibly to the demands of the post. Work as a member of a team. Show a capacity to work alone and the ability to keep calm under pressure. Show commitment to facilitating positive outcomes for service users Desirable criteria: A recognised professional qualification e.g. DipSW, Dip Counselling, Dip Addiction, Youth Justice Board, Health and Social Care NVQ 3 or a qualification in Youth and Community work.
