the Sacraments to be celebrated in the parish in which the family resides – not according to
which school your child attends. The Sacramental Programme within the Archdiocese is
parish based, family centred and school supported.

 Please attach copy of Baptism Certificate to this application. No applications will be
accepted without this Certificate. Duplicate copies of certificates for children baptised
within this Parish are available for $5 by applying in writing (incl email) and
collecting/paying in person at the Cathedral Office by Monday 13th October.
 Please circle Sacramental preparation sessions most convenient : 3.30pm (weekdays
TBA); or 5.00pm Sunday (final days will be emailed to you when applications close)
 Please post or deliver this application form in person to Cathedral Office along with the $30
application fee (covers paperwork, study book, certificate, communications etc). No
applications will be accepted without this payment.
