Adopting an IEP from another district – Process:
-Obtain IEP and ETR from former school
-Amend front page of the IEP by creating a new RIEP in Progress book
-On front page of the new IEP, state in other information “The team agrees to adopt the
IEP for a period of no longer than 30 days and will reconvene to conduct a new IEP
annual review. The IEP team also agrees to adopt the Evaluation Team Report (ETR)
from the previous school.”
-If you make any changes to the IEP please note these on the appropriate page and make
a note on the amendment section of the front page
-Attach the following forms along with the complete IEP from former school:
-Front page with statement in other information
-New team participation page
-Parent signature page
-PR-01 (new version posted on website specifically for transfer IEP)
-Parent Invitation (add student name if over 14)
-EMIS form (use current date as start date, end date is the date the IEP expires
from the current school)
-Make copies and send all original forms to the main office
