Checklist for Fifth Year Scholars

I have read and understand the Fifth Year Scholar Program Policies & Guidelines
document and the stipulations with regard to academic arrangements and nonacademic considerations.
I understand that the Associate Dean or other designate in my college will serve as
my academic advisor for my fifth year and have discussed my plan of study with this
individual.
I understand that I must have completed my primary degree requirements before
the fall of the 2013 academic year.
I understand that I must determine the potential cost of courses or fees associated
with my Fifth Year experience and not covered by regular tuition and request
approval for this funding by November 5, 2012.
I agree to meet with my project advisor by October 5, 2012 to set a timeline to work
on my community impact proposal.
Upon reviewing the program Policies & Procedures, as well as the above checklist, I
accept the Fifth Year Scholarship offer for the 2013-2014 academic year at Carnegie
Mellon University.
