Note: Please use this checklist during drills and emergency evacuations as
guidelines to follow. Submit completed checklist and Floor Warden checklists to
the Zone Captain for documentation.

1. Don vest. Grab additional emergency equipment.
2. Confirm need to evacuate and initiate evacuation process.
Activate Floor Wardens.
3. Establish contact with ZC at the zone command post or other
designated location.
4. Supervise building evacuation to primary Assembly Area. If
primary location or route is unsafe, direct evacuees to
alternate Assembly Area. Advise ZC if alternate area is used.
5. Receive preliminary reports from Floor Wardens.
6. Summarize reports and submit status to ZC:
General building condition
Number, location and condition of individuals remaining in
building
Location of recognized hazardous conditions (fire, hazmat,
structural problems, falling hazards, etc.)
7. Select recorder to maintain an information log tracking all
events (who, when, what, where).
8. Select dedicated runner(s) for messages.

9. Look after safety of evacuees in the Assembly Area. Request
resources from ZC as necessary.
10. Do not allow anyone to re-enter the building if any unsafe
condition is suspected, or as directed by emergency
responders.
11. Maintain communications with ZC and FW regarding status of
event. Work with FW to keep evacuees informed as much as
possible.
12. Coordinate re-entry of building when “all-clear” notice is given
by Zone Captain or other designated emergency responder.
13. Request release of duty from the ZC. If emergency is
continuing, hand-off vest and other equipment to replacement
and provide briefing.
14. Post-event – Document any other observations related to this
incident worth reporting.
15. Post-event – Participate in debriefing and development of
lessons learned.
16. Post-event - Replace vest, emergency equipment. Replenish
supplies as necessary.
