SOUTH EAST SCOTLAND SUB-SPECIALTY SPR TRAINING IN HEPATOLOGY JOB DESCRIPTION This post is based in the Scottish Liver Transplant Unit and the Liver Unit of the Royal Infirmary of Edinburgh and will be for the duration of 12 months. It is designed for established Specialist Registrars in Gastroenterology who wish to gain sub-specialty training in Hepatology. SCOTTISH LIVER TRANSPLANT UNIT The Scottish Liver Transplant Unit is the National Unit for Liver Transplantation in Scotland and also a National Referral Centre for Patients with acute liver failure. The Unit undertakes approximately 90 liver transplants per year. Consultant Hepatologists: Dr A J MacGilchrist Professor P C Hayes Dr K J Simpson Prof J N Plevris Dr C Blair Dr Andrew Bathgate Prof Stuart Forbes Other Staff Two-four Specialist Registrar Posts One University Lecturer One Additional Visiting Registrar Two full time and one part time Staff Grade/Assoc specialist Two Senior House Officers/FY2 Academic fellows who help with on call Clinical Duties 1. Day to day care of patients in the Scottish Liver Transplant Unit (Ward 206) as well as patients in Liver Transplant High Dependency Unit and care of the liver transplant patients in the Intensive care unit. 2. Contribution to the Management of patients admitted under the care of consultant Hepatologists to the Medical Day Case Unit. 3. One to two sessions weekly for gastrointestinal procedures. 4. One to two out-patient clinics. 5. Emergency endoscopies as required. 6. Contribution to new review liver transplant and Hepatology clinics. 7. Participation in academic, clinical and audit meetings. Liver Unit During the 12-month attachment 6 months will be in the Liver Unit. This is primarily based in Ward 205 which is a 28 bedded ward is looking after patients primarily with Liver disease referred both local and tertiary referrals. The senior medical staff are the same as those for the National Scottish liver Transplant Unit with the addition of Drs Kenneth Trimble, Ian Penman, Norma McAvoy and Nick Church, luminal Gastroenterologists and Dr Neil Henderson, consultant Hepatologist. Middle grade staff similarly are the same with different Senior House Officers/FY2s and Pre-registration House Officers. During the attachment there will be more endoscopic and out patient clinics than when attached to the Scottish liver Transplantation. Rotation through portal hypertension and the hepatitis C Clinics will be involved. It is hoped that during this attachment there will be opportunities for more specialist training such as wedged hepatic venous pressure measurement, transjugular biopsies and perhaps TIPSS. Teaching Commitments The Specialist Registrar will be required to participate in teaching undergraduate and postgraduate students. Research Comprehensive research facilities are available within the University Liver and Gastroenterology Department. The Specialist Registrar will be given the every encouragement to pursue research work appropriate to his or her career intentions. The new Trust Clinical Research Facilities have improved clinical research opportunities. Administration There are no administrator duties specifically related to this post. However, the Specialist Registrar may be required to contribute to the internal organisation of the Liver Unit and the Scottish liver Transplant Unit. Training Opportunities A Wide Range of postgraduate educational facilities are available within the Lothian Area, in addition to the training experience and clinical meetings available directly in the Liver Unit and the Scottish Liver Transplant Unit of the Royal Infirmary of Edinburgh. On-Call The on-call duties for ward emergencies involve a rota of approximately one in five. The contract for the post is currently 40 standard hours per week. The holder of this post will provide mutual cover as required for the annual and study leave of colleagues. He/she will be prepared to perform duties in the occasional emergencies and unforeseen circumstances without additional remuneration but may be granted time off in lieu at the discretion of the employing authority. Commitments arising in such circumstances are however, exceptional and practitioners should not be required to undertake work of this kind for prolonged periods or on a regular basis.
