TRANSFERS TRANSFERS FROM WITHIN THE SCHOOL SYSTEM: Students who transfer from within the school system should immediately be placed for services as indicated on the IEP. Any changes to the IEP require an IEP Team Meeting. When a student is transferring from your school, it is the responsibility of the resource teacher to: A. Notify the Special Education Office personnel by E-mailing Denise Cannon information regarding location of transfer. Attach note to front of the student’s file, and give it to the school psychologist for transfer to new school. B. Denise Cannon will change Easy IEP Access rights to the personnel of the receiving school. TRANSFERS FROM OUTSIDE THE STATE: A. If you do not have records, get the parent’s signature on a Release of Information form and send for the records. In most schools the attendance secretary takes care of this. B. Once you have received the student’s records, notify your school psychologist for reviewing. C. After the records are reviewed, you will be notified to have an initial IEP Team meeting and whether additional testing, etc. is necessary. In other words, if everything is complete and current according to state guidelines, AIP procedures or testing, etc. You would simply have an initial Team meeting using transfer records. However, the school psychologist or speech therapist will need to review the records and write a new eligibility report. We do not want you to do anything unnecessarily. If however, the review of records indicted the student needs to be re-evaluated, you will be notified to begin the AIP process as soon as possible. Do not take students on parents’ verbal report or verbal report from schools. Contact schools and have them FAX the IEP immediately and mail the rest to the Special Education office. TRANSFERS FROM OUTSIDE OF THE SYSTEM BY INSIDE THE STATE: A. Send the student’s identifying information and the name of the previous system and school name to Denise Cannon. She will attempt to upload the IEP via Easy IEP. B. The Uploaded IEP must be changed to reflect the student’s new address and school assignment. All start dates must be changed to the date the student begins special education services within Wilson County. A meeting must be held and placement signatures received. C. As full records are received they should be given to your school psychologist for review and confirmation of eligibility. D. Once the psychologist/SLP confirms eligibility be sure to enter correct enrollment/eligibility dates and finalize the IEP. TRANSFERS OUT OF THE SYSTEM: Once a request from the child’s new school system has been received, copies of the file are sent to the new school via fax or mail. This request may come by phone to either the individual school or to the Central Office. Please forward the request to be handled by the records secretary at the Central Office. Before a file can be sent to the new system, a formal request on the system’s letterhead must be received to be kept in Wilson County’s copy of the student’s file.
