TieNet Amended IEP (11/12/08) What? o The “Amended” IEP is a regular IEP with a checkbox and date that indicates it is an “amended” IEP. o This is NOT a separate document for IEP amendments. Why? o Document IEP changes between Annual Reviews Casemanager, Services, Assessment, etc o Maintain accurate data for yourself and others Most Reports are based on data in the Student Demographic Profile o Document Compliance/monitoring o State Reporting December 1 Count, **Primary Disability, IEP date, EE setting State Assessment (Dec 31), etc When? o Whenever significant changes are made regarding the IEP o REMEMBER: IEP documents are to be finalized within10 working days of the meeting or action. o Prior to state reporting deadlines Which Students o Any student who has had changes in programming regarding the IEP REMEMBER: This includes changes in Assessment, since those questions changed this fall. o Any student who has NOT had an Annual Review since the “Migrated” IEP o Check Student Demographic Profile Eligibility, Assessment o Ad Hoc Reports may be useful in checking your current information. Public Reports How? o Complete appropriate Prior Notice process (P. 71 Procedure Manual) o Have meeting, as necessary, to make decision, inform parents, etc. o Select student and choose New, age appropriate, IEP document o REMEMBER: Put information in “COMMENTS” regarding purpose of the “amendment”. o Copy All from Previous IEP (even if only changing 1 section) it is important to keep all the current information in one place o Check “Amendment to IEP” on cover page and enter date of amendment o **Make sure the “IEP Meeting Date” still correctly reflects the last IEP meeting date Initial/Annual Review. (NOT the amendment date) o Make changes to sections as appropriate. o Finalize the document to “push” current data to the Student Demographic Profile o Print the Cover Page with Amendment date, and only the page(s) to which changes have been made. a. Give parents copy and add ONLY the appropriate Amended IEP pages to the current IEP in the Cumulative folder (including Amended Cover Page). i. Do NOT remove previous page that has been amended, as the historical information may be needed.
