Evaluate is received)

to Evaluate no later than 2 weeks after the Referral Packet is received by Sped Services. Case
Manager sends home the Consent form with the child or mails it and notifies parent that the
form is being sent home.
 Exception: The date of the Eligibility Consideration Meeting is on or after April 1 st.
5.
If the parent is NOT present at the meeting, the Case Manager and Psychologist schedule a date for testing and the
Case Manager contacts the parent to secure the signed parent Consent to Evaluate.
6. Case manager date stamps the Consent to Evaluate when parent returns it and sends
