After the last Tier III Interventions denote a need for Special Education Consideration, a Case Manager is assigned at the school and the Case Manager confers with the psychologist and/ or SLP to schedule the Referral to Special Education meeting. Send Notifications / Reponses and Parental Rights (SPE 87) to parents, inviting them to the meeting to discuss and plan necessary evaluations. Notification of the meeting should be no less than 7 calendar days prior to the meeting. 2. Notify IEP Team: classroom teacher (s), guidance (if ED or +14), special ed teacher, speech (as needed) and LEA rep( administrator, additional special ed teacher, regular, or others with administration certificate). Other relevant personnel (nurse) may be included. 2nd notification sent 3 DAYS prior to meeting. 3. Case manager assigns a scribe at the meeting to write the Minutes/PWN. (SPE 21R) 4. If parent is present at the Eligibility Consideration Meeting and agrees to the evaluations they MAY sign the Consent to Evaluate, the PSYCHOLOGIST / and or SLP, along with the team, schedules a date for the Evaluation and the follow-up eligibility meeting. (50 calendar days from the date stamp on the consent – when parent Consent to Evaluate is received) 2/26/14 NEW: …….OR, the PSYCHOLOGIST notifies the Case Manager when to send the parent the Consent to Evaluate no later than 2 weeks after the Referral Packet is received by Sped Services. Case Manager sends home the Consent form with the child or mails it and notifies parent that the form is being sent home. Exception: The date of the Eligibility Consideration Meeting is on or after April 1 st. 5. If the parent is NOT present at the meeting, the Case Manager and Psychologist schedule a date for testing and the Case Manager contacts the parent to secure the signed parent Consent to Evaluate. 6. Case manager date stamps the Consent to Evaluate when parent returns it and sends ORIGINAL to Sped Services the same day and a COPY to the Psychologist. 7. The Psychologist/SLP schedules the M-Team Meeting with the Case Manager 50 days out from the date stamp on the signed consent. 03/14 Step 2 1. The committee reviews evaluation results and other relevant information and determines eligibility for Special Education Services. The Multi-Disciplinary Team Report is completed and signed. (SPE-24R) 2. Parent signs Excent Parental Consent for Placement. 3. The DRAFT IEP is presented, discussed and signed by the committee members. 4. Committee sign the Minutes/PWN. (SPE 21R) 5. Parent receives a copy of: IEP, Multi-Disciplinary Team Report (SPE-24R), Minutes/PWN (SPE 21R) and all evaluations used to determine eligibility. 6. HS ONLY: Complete / Sign SPE-28 for RATE Alternative Credential Students 7. Case Manager completes the Special Education Status Report (SPE 26), gives a copy to attendance secretary and special services secretary to ensure correct database information. 8. Send Packet: Originals of the IEP, and all forms listed above to the secretary of special services within 2 weeks of the meeting date. 9. If RTI/ Sped Referral forms, and MDC 1, SPE8,9,,56,67 were already sent through RTI-Team, do not re-submit) NOTE: If parent does not attend and has indicated intention to attend, conduct the meeting and have parents sign Excent Parent Consent to Place at a later date. Parent Signature must be obtained prior to initiating services on the IEP.
