He/she is a flexible, empathetic counseling professional who will employ various evidenced-based behavioral health techniques when working with the people we serve, who largely include those who are homeless and/or extremely low income. This person will be a part of a team implementing a federal grant called Hurry Home. The purpose of Hurry Home is to assist chronically homeless people with mental health and substance abuse disorders access permanent housing with agency support with a multi-disciplinary team. The counselor also will provide assessment and counseling to patients within our clinic as referred by our medical provider. The behavioral health counselor serves at the behest of St. Luke’s Chief Operating Officer and is an at-will employee. Major Functions and Responsibilities: The behavioral health counselor will assess and diagnose potential candidates for the Hurry Home program, develop a behavioral health plan for program participants, and provide direct counseling, group therapy, and support groups. This person will also work with St. Luke’s staff on facilitating the integration of behavioral health care into the clinic’s primary care practice. The behavioral health counselor actively engages with others and assures quality improvement planning is ongoing, updated, and capable of producing evaluative measures and reports. Major Activities and End Results: 1. Provide direct patient screening, assessment, and therapy to homeless and indigent adults and children. 2. Coordinate formal/informal mental health and substance abuse training for staff, providing programs to address the various behavioral health issues afflicting patients. 3. Develop a crisis response plan to handle crises within the clinic and its property. 4. Help develop comprehensive delivery service systems, in collaboration with SLHCC’s Medical Director, for primary care patients with behavioral health needs. This means developing evidencebased screening and assessment tools appropriate to St. Luke’s and its patient population, as well as providing training to all clinical employees. 5. Help develop counseling program targeting needs of seasonal agricultural or migrant workers. 6. Keep abreast of behavioral health trends of high-risk populations such as those relevant to homeless or economically disadvantaged people. 7. Comply with quality assurance/quality improvement plans and activities, producing reports for quality improvement. 8. Coordinate with other clinical staff and management on program planning and evaluation. 9. Help coordinate programs associated with behavioral health to meet grant program requirements. Experience, Knowledge and Skills Minimum Qualifications: Independently licensed counselor (LISW or LPCC) or within six months of independent licensure. Experience working with under-served populations, providing behavioral health and substance use services to people who are low-income and/or homeless. Desired Qualifications: Bilingual or Spanish-English proficiency working with multiple evidencebased practices for people experiencing depression, trauma, or psychosocial disorders Education, Credentials, Licenses Minimum Qualifications: LISW, LPCC with current license to practice in New Mexico or within six months of licensure. An LMHC or LADAC with experience may also be considered. Other: Position will involve occasional state and out-of-state travel for clinic specific training and meetings.
