Job Summary:
The Nurse will work as part of the Practice
Nurse Team, taking a key role in promoting
health and delivering health care to patients
who are registered at Combs Ford Surgery.
The Nurse will take part in the ongoing
development of a team approach to patient’s
health and well being, embracing the changing
role of the Practice Nurse, in partnership with
patients and other staff at the practice and in
the community and secondary care.
Principal Activities :
To work as part of the Nursing Team including Saturday rota, sickness and
holiday cover in addition to contracted hours.
1. To receive and treat casualties, using own experience and judgement, and
calling on medical advice when necessary.

2. To carry out vaccinations and immunisations on both children and adults,
in accordance with agreed local patient group directions and national
guidelines
3. To keep clear and accurate records of treatment and observations made
of patients, using the practice computerised clinical system, in accordance
with NMC guidelines and practice policies.
4. To organise and run the Chronic Disease Management Clinics in
conjunction with other members of the nursing and medical team, using
current evidence based information and guidance. Chronic disease
includes Diabetes, COPD, Asthma, Hypertension, CHD, Stroke and any
other registers included in Quality and Outcome Framework.
5. To provide the best possible support for patients with chronic disease in
order that they can improve their health using current Health Promotion
literature, self help groups as required by NHS Suffolk.
6. To work with other members of the Primary Care Team on joint projects
for the benefit of patients i.e. Stop Smoking Clinic, Chlamydia Screening.
7. To recognise the need for professional development and identify own
education and development needs, participate in further education
programmes, study days and read appropriate literature to maintain
knowledge of current practice and complete an up to date portfolio of
experiences to fulfil the requirements of PREP.
8. To undertake regular Clinical Supervision and Appraisal.
9. To be aware of current infection control guidelines, and the correct
procedures for the disposal of hazardous waste, including sharps bins and
clinical waste.
10. To be aware of responsibilities for self and others under the EU Moving
and Handling legislation and Health and Safety at Work Act and ensure
regular attendance at training and updating sessions.
11. To be fully conversant with Suffolk Child Protection policies and
attend/complete annual updates and training in this and resuscitation and
anaphylaxis.
12. To identify training needs of self and other team members in partnership
with Clinical Nurse Practitioner and Practice Manager and participate in
the appraisal system.

financial instructions, policies, procedures and standards of Combs Ford
Surgery, as well as legislation and professional standards and guidelines.

TRAINING
It will be necessary from time to time to attend external and/or internal training
sessions and meetings on a formal basis. It is also the responsibility of the
individual to maintain a high standard of personal knowledge and understanding
of their role.
APPRAISAL
To attend appraisal and with supervisor to formulate a development plan.
All practice staff should be aware of their responsibilities under the Health and
Safety Act 1974 and Data Protection Act 1988, The Access to Medical Records
Act 1986 and the Access To Health Records Act 1990 and subsequent
amendments.
This job description will be subject of annual review and amendment in
conjunction with the post holder.

Person Specification:
An RGN with a minimum of 3 years experience, holding a valid NMC
Registration, who has a variety of experiences in Chronic Disease Management,
Travel Health and Childhood Immunisations and has the ability to carry out duties
of patient care in the Treatment Room.
