What’s Changed? Transitioning from Niday to BORN In response to your requests, data elements have been added/modified to help clinical decision making and to inform practice at your organization (i.e. Bishop score, Primary indication for Induction of labour, newborn complications) Subject Matter Expert (SME) working groups reviewed and updated data elements to ensure consistency with current clinical practice, best practice guidelines and changing trends in population health. Select data elements have been re-organized and/or singled out to increase specificity, improve data quality and ease of analysis (i.e. mental health concerns, diabetes in pregnancy) Redundant data elements have been eliminated (i.e. ‘Early Attachment/Feeding Initiation’ replaces ‘Infant Feeding in Hospital’) Manual Data Entry Sites Data elements are listed in the quick reference guide (table below) For a more detailed document detailing the relationship between Niday and BORN Ontario data elements, please refer to the companion document (excel spreadsheet) sent electronically with this guide For the complete data set, please refer to the online Data Dictionary www.bornontario.ca/data-dictionary Electronic uploading hospital sites For BORN hospital upload file specifications, Frequently Asked Questions pertaining to the upload process, etc. please refer to: www.bornontario.ca/data/information-technology/born-build QUICK REFERENCE GUIDE NEW Data Elements Name OHIP # ID Type Unit # & Street Telephone Woman Abuse Ancestry Field Conformance Rationale M= Mandatory, C= Conditional, O = Optional M C C O O M O Continuum linking and matching Population health risk factor Population health risk factor 1785 Alta Vista Dr. | Suite 106 | Ottawa, ON K1G 3Y6 | P: 613.523.3781 | F: 613.523.9057 | www.BORNOntario.ca 1 NEW Data Elements Pregnancy History EDB EDB Determined by Gravida # of Previous Abortions (A) # of Living Children (L) # of Previous Stillbirths (S) # of Previous Vaginal Births # of previous VBACs Maternal Weight at end of pregnancy Folic Acid Oncology Radiation Therapy Exposure in Pregnancy Intrapartum Admission by Healthcare Provider Primary Indication for Induction of Labour Bishop Score If C/S, Stage Primary indication for Caesarean Section Anesthesia for Caesarean Section Postpartum Maternal Outcome Maternal Transfer To Reason for Maternal Transfer To Maternal Discharge Date/Time from Hospital Barrier to Maternal Transfer Postpartum Complications Postpartum Breastfeeding Support Feeding method Field Conformance Rationale M= Mandatory, C= Conditional, O = Optional M M M M M M M M M M Alignment with antenatal one form Enhanced obstetrical history Health status in pregnancy Current practice standard C Risk exposure in pregnancy M C C C C c Resource utilization analysis/transfer of care To improve accuracy Clinical practice standard Enhanced risk assessment To improve accuracy Improving accuracy/enhanced risk assessment M C C M C M C O Evaluate maternal outcomes Birth Order C Date/Time of Newborn Screen Collection C Service utilization Evaluate maternal outcomes Current practice standards / BFI accreditation Linking and matching, enhancing birth outcomes analysis (i.e. multiples) Current practice standard - reconcile with Newborn Screening Ontario lab results 1785 Alta Vista Dr. | Suite 106 | Ottawa, ON K1G 3Y6 | P: 613.523.3781 | F: 613.523.9057 | www.BORNOntario.ca 2 CHANGED Data Elements Province Primary Language Aboriginal Identity Pregnancy History Pre-existing health conditions # of fetuses Type of Conception Mental Health Concerns Smoking at first prenatal visit Resides with smoker at first prenatal visit Smoking at time of admission for birth Resides with smoker at time of admission Alcohol Exposure in Pregnancy Drug and Substance Exposure in Pregnancy Medication exposure in pregnancy Infection & Pregnancy Diabetes and Pregnancy Hypertension Disorder in Pregnancy GBS Screening/Results Intrapartum Cervical Ripening Birth Location Newborn Resuscitation Pregnancy Outcome Neonatal Death Head Circumference at Birth Postpartum Arterial cord blood test status Venous cord blood test status Newborn Congenital Anomalies Suspected/Confirmed Newborn screening Breastmilk Substitute Infant Early attachment/feeding initiation Newborn Discharged or Transferred to Neonatal Transfer to NICU/SCN Description of Change/Rationale Separated NWT & Nunavut Updated and more specific More specific / updated options Re-organized by system Replaces 'multiple gestation' Replaces 'assisted reproductive technology' Moved from 'pre-existing health conditions' Enhanced risk assessment Moved from 'pre-existing health conditions', enhanced risk assessment Moved from 'complications in pregnancy’, enhanced risk assessment Increased specificity;
