MIDDLE/HIGH SCHOOL COLLABORATIVES FOR TRANSITION FROM MIDDLE TO HIGH SCHOOL MIDDLE SCHOOL RESPONSIBILITIES: For the initial collaborative meeting, compile packet of information to include: o Attendance records o Behavior records o Copy of Transcript o Current class schedule with current EXED services indicated o Taking/passing Physical Science and/or Algebra I o Eligible to take the Algebra I EOC exam o Copy of completed high school registration form (if available) Collaborate with the high school to develop transition plan: o Middle school should complete the “Measurable Post Secondary Goals” by direct interview with the student prior to collaboration Collaborate with the high school in the development of appropriate and measurable goals and objectives based on academic and post-high school goals. (Brainstorm goals for Academics, Behavioral & Vocational/Transitional) In EasyIEP, complete the Assessments, Narratives, and Participations tabs. Collaborate with the high school staff regarding accommodations for high school courses. Interview parent regarding concerns about the transition from middle school to high school and place that information under the Parent Concerns in the Narratives. Early in the collaborative process, the High School needs to be informed by the Middle School of any students who will be getting ESY services during the summer of 2010. HIGH SCHOOL RESPONSIBILITIES: Collaborate with the middle school to develop the appropriate and measurable goals and objectives based on CURRENT information about the student’s present levels of functioning. Collaborate with the Middle School to determine how the student’s disability affects involvement and progress in the general curriculum. Collaborate with the middle school to develop the transition plan: o “Transition Services Planning” Measurable Post Secondary Goals and Course of Study (by age 14) o “Transition Services” (by age 16) developed based on the “Measurable Post Secondary Goals” Complete the “Services” page in EasyIEP Collaborate with the middle school to complete the Accommodations, and Participations tabs in EasyIEP Under the Participations Tab, be sure to address the State/District Mandated Tests and either allowable and/or special accommodations. If there is a need for a Behavior Plan, be sure that Behavior is addressed in the Consideration of Special Factors under the Narratives Tab in item #6. Behavior plans must include goal/objectives to reasonably ensure success. Accommodations for behavior should be addressed on the Program Participation under “Behavior Accommodations” or “Custom Accommodations”.
