POST INFORMATION
This is a new/replacement post to join a team of 5 Respiratory Specialist Physicians, based
at James Cook University Hospital It is advertised on a whole time (10 PA basis), although
any candidate who is unable to work full time will be eligible to apply and, if appointed,
modification of the job content will be discussed in consultation with consultant colleagues
and the Medical Director.

South Tees Hospitals NHS Foundation Trust is the largest hospital trust in the Tees Valley
serving the people of Middlesbrough, Redcar and Cleveland and Hambleton and
Richmondshire and beyond and provides health care from two main sites – The James Cook
University Hospital in Middlesbrough and the Friarage Hospital in Northallerton.

The organisational structure comprises of the Board of Directors with the Chief Executive,
Trust Chair, Corporate Directors and Non-executive Directors with a Management Group
made up of the Chief Executive, Corporate Directors and Chiefs of clinical divisions which
are Acute Medicine, Specialty Medicine, Surgery, Women and Children, Trauma,
Cardiothoracic, Neurosciences, Anaesthesia and Theatres, Radiology and Pathology. There
is a separate Academic Division with its own Chief of Service and a Division of Clinical
Support Services.
The Trust is part of the recently formed North of England Cancer Network which is made up
of the old Northern Cancer Network and the Cancer Care Alliance.
The Regional Spinal Injuries Unit provides a comprehensive spinal cord injury service for the
North of England (population 3.4 million).
It also covers a range of highly specialist tertiary services extending to 1.5 million people in
Teesside and parts of Cumbria, Durham and North Yorkshire with leading expertise in heart
disease, cancer, trauma, neurosciences, renal services and spinal injuries.
The Trust’s core business is to provide secondary healthcare to:

Around 274,000 people living in Middlesbrough and the local area of Redcar and
Cleveland
124,000 people in an area stretching from the North Yorkshire Moors to the central
Pennines, the borders of York district in the south and the borders of Darlington in the
west.
In 2009/10 the Trust’s budget is £418.1m and it employs almost 7,000 full and part time staff.

Hospital at Night
The Trust has developed an at night wireless communication system between ward and
medical staff via a night co-ordinator. This allows on-call clinicians receiving ward requests
for help to prioritise calls, using supplied clinical observations, through a PDA, gives access
to protocols on patient management and also allows data collection on patient and ward
activity and response times to seeing patients and completing tasks. This system has been
awarded the prestigious HSJ Award for e-technology and has also won a BT award.
In January 2006 the Trust achieved Practice Plus status for work on Improving Working
Lives. This is an important recognition for commitment to good working practices and, as
equally important, how staff find working in the Trust.

THE JAMES COOK UNIVERSITY HOSPITAL
One of Europe’s most modern hospitals, The James Cook University site is the largest of its
type and has a wide range of specialities. This hospital was opened in October 2003 and has a
capacity of 1,007 beds. Based on the original site of South Cleveland Hospital, all acute and
elective medical and surgical specialties are accommodated. Tertiary services, in addition to
cardiology and cardiac surgery, include renal medicine/haemodialysis, neurosurgery, vascular
surgery and the regional spinal injuries unit.
This site contains a modern central operating suite containing 22 well-equipped theatres,
with two recovery areas. This is immediately adjacent to an Adult Surgical Day Unit
containing a further three theatres and also to the Paediatric Day Unit. The 16 bedded
general ICU is across the corridor. Two additional theatres for gynaecology and obstetrics
are in the Central Delivery Suite and two more in the Ophthalmology Day Unit.
The Accident and Emergency Department on this site is the designated Regional Trauma
Centre for Teesside, South Durham and North Yorkshire. The hospital has a 24-hour Acute
Admissions Unit.
There is a 21 room imaging department equipped with a full range of mainly digital
modalities, a trust-wide PACS system and a purpose built 6 bed Radiology Day Unit. A
Medical Physics Department is also on this site together with isotope imaging.
The central clinical laboratory provides a comprehensive pathology service with electronic
links to wards and departments. Electronic requesting is being piloted.
There is a purpose-built Endoscopy centre.
The hospital has a comprehensive tertiary level fertility service and houses the Gynaecology
Cancer Centre (North of England Cancer Network).
The hospital caters for more than 4000 deliveries in purpose built delivery and low
dependency suites. The birth rate is increasing. Fetal medicine and high dependency
delivery referrals are accepted.
There is a regional tertiary neonatal intensive care unit with 9 intensive cots, which accepts
both local and regional admissions. It supports a neonatal transport service and has a strong
academic record. Any babies born at the Friarage hospital (c1100 deliveries per year)
requiring intensive care are transferred to this unit.
There is an established Academic Centre embracing postgraduate medical education, Nurse
Training School, the School of Radiography and library facilities. It encompasses two large
lecture theatres with the latest audio-visual aids and a considerable number of classrooms and
seminar rooms, including ward facilities for teaching and examining. There is a new library with
comprehensive IT suite.
A dedicated undergraduate teaching centre has been commissioned recently and there are
plans to have a dedicated simulation-training centre on site in the near future.

An accredited major trauma receiving hospital, it contributes to UK Trauma, Audit &
Research Network (UK TARN). It occupies a leading UK position in its survival rates from
blunt trauma as evidenced from UK TARN.
THE FRIARAGE HOSPITAL
The Friarage Hospital at Northallerton, situated approximately 23 miles from The James Cook
site, merged with South Tees Hospitals NHS Foundation Trust in April 2002. It provides a wide
range of secondary services to its local population and is well known for its friendly atmosphere.
A fully equipped district general hospital with 314 beds, it provides a 24-hour accident &
emergency service and offers inpatient and outpatient facilities in general surgery including
colorectal, breast (including mobile screening) and urology services, orthopaedics and
trauma. Medical specialties include acute medicine, diabetes and endocrinology,
rheumatology, gastroenterology, respiratory and rehabilitation medicine. Inpatient and
outpatient obstetrics/gynaecology, paediatrics and adult psychiatry services are on site
Cardiology, encompassing CCU and cardiography, functions as a self-contained unit but is part
of the cardiology directorate. There are two cardiologists in post. Increasing integration of the
cardiology services across the two sites is planned.
The hospital is furnished with a well-equipped medical library with facilities for electronic
searching of medical databases and access to the Internet. A full programme of
postgraduate educational meetings is held on this site.
Support services on this site include haematology, chemical pathology, histopathology,
microbiology, physiotherapy, occupational therapy and speech and language therapy.
Patients requiring cardiothoracic or neurosurgical services, nuclear medicine or MRI
scanning are referred to other hospitals including The James Cook University Hospital and
Leeds General Infirmary (45 miles).
The Ministry of Defence Hospital Unit (MDHU) has been based at the Friarage since July
1999 with contribution of military doctors, nurses and other staff who are integrated into the
hospital’s services.
EDUCATION, TEACHING AND RESEARCH
The modern Academic Centre at The James Cook University Hospital is housed within two
linked buildings. They contain two large lecture theatres, which have audio-visual links.
There are many seminar rooms of varying sizes, which are used for teaching,
demonstrations, lectures and conferences. There is a new library which is well equipped
with most of the general and specialist journals relevant to the Trust’s clinical activity. There
is a large IT suite of over 40 stations within the library complex.
The Academic Centre also houses the Clinical Trials Unit which has recently been extended
with links to the unit at the Royal Victoria Infirmary in Newcastle. Clinicians who are
interested in developing a clinical trials programme are encouraged to contact the Research
& Development Department.
There are close links with the College of Health at Teesside University for the training of
nurses and midwives, radiographers and physiotherapists.

There is an active postgraduate district and divisional curriculum on site based in the
postgraduate education centre under the direction of a university clinical tutor. There is an upto-date medical library within this centre. Consultants are expected to undertake activities
suitable for continuing medical education in accordance with Royal Colleges.
As part of the national expansion of medical student numbers, the Universities of Newcastle
and Durham have formed a partnership increasing the undergraduate intake to 340 students
as from September 2002. The first two years of the curriculum are being taught at the
Queen’s Campus of the University of Durham in Stockton as well as the Medical School in
Newcastle. Currently it is planned that 102 students will be based in Stockton and 245 in
Newcastle including 25 in the accelerated programme. At the end of year 2 the entire
cohort of students will be divided up for their third year between base units of acute and
community trusts and PCTs around the Northern Region. South Tees has been designated
the lead trust for the Teesside Unit which currently comprises Bishop Auckland and
Darlington, North Tees and Hartlepool, as well as South Tees and the corresponding Mental
Health Trusts of Tees, Wear and Esk Valley and South Durham and the associated PCTs.
In order to accommodate these medical students, a new purpose-built clinical Academic
Centre, including a teaching unit, has recently been completed at The James Cook
University Hospital. There are active plans to further develop both teaching and research
within the Trust including the appointment of a new Chair in Rheumatology (Newcastle) and
Cardiology (Durham).
Research is actively encouraged. The Trust employs several visiting Professors (Orthopaedics
(2), Anaesthetics (2), Academic Emergency Medicine (1), Paediatrics (1), Medicine (1), Surgery
(1) and Maxillofacial Surgery (1). The Sub-Dean of the Medical School (Professor Bilous) is
also based at JCUH.
As a result of these activities the Trust was designated a University Hospital in 2002.
OTHER FACILITIES
The James Cook University Hospital has an on site family friendly crèche open from 0730
until 1800hrs.

THE SPECIALTY RESPIRATORY MEDICINE
DIRECTORATES OF RESPIRATORY MEDICINE
DEPARTMENT
Wards 9 and 10 JCUH
This is a 59 bedded Respiratory Medicine Unit. Ward 9 is Female Respiratory Medicine and
Ward 10 Male Respiratory Medicine. Both wards have side wards for nursing patients with
Pulmonary TB or terminally ill patients. The unit cares for patients requiring Non invasive
Ventilation (NIV) In 2010 one of the 6 bedded bays on Ward 9 will be re-designed to become
a 4 bedded palliative care bay. Patients are admitted to the unit via AAU, A&E and from
Outpatients.
Respiratory Function Lab
This laboratory is close to ward 9 and staffed by a Senior Lung Function Technician and a
Trainee who on completion of her training will increase this qualified establishment to 2.0
wte. All routine lung function tests are performed together with bronchial challenge testing
and arterial blood gas analysis.
Fibre-optic Bronchoscopy
Currently there are two sessions each week in the Endoscopy Centre. The majority of cases
attend as a day case. Video and direct viewing bronchscopes are available with video
recording and photography routinely used. An image intensifier is available if required for
transbronchial biopsy.
Intralunminal Brachytherapy
This service is currently run by Dr N Krishnaraj, Respiratory Consultant and Dr N Wadd,
Consultant Oncologist on an approximately bi-weekly basis as needed. A purpose built
brachytherapy room in the Radiotherapy Centre is used for the intraluminal treatment.
COPD Outreach and Oxygen Assessment Service
Four full-time and one part-time Specialist Nurses run both the COPD Out reach and
Oxygen Assessment Services with Physiotherapy input for COPD. These services support
the both the respiratory and other acute medical wards giving advice on early supported
discharge and home oxygen requirements as applicable. COPD patients are followed up in
their own homes for up to a week after discharge.
Pulmonary Rehabilitation
A well organised Pulmonary Rehabilitation Service is run by our Senior Physiotherapist with
classes for groups of patients with COPD held weekly.
Lung Cancer Support Team
Three Specialist Lung Cancer Nurses, an Occupational Therapist, Social Worker and Data
Coordinator offer support at all stages of the patient journey through diagnosis, treatment

and terminal illness. There are close contacts with the Community bases Macmillan Nursing
Service.
Multidisciplinary Team Meetings for Lung Cancer
There is a monthly MDT involving Cardiothoracic Surgeons, Radiologists, Pathologists,
Physicians, Specialist Nurses and representatives from the Teesside Cancer Alliance, for
patients being considered for surgery. A weekly unit meeting is also held to discuss all other
patients with Lung Cancer.
Pulmonary Tuberculosis
The Respiratory Directorate is responsible for the diagnosis, treatment and follow-up of
patients with Pulmonary TB. There are two Specialist Nurses who supervise chemotherapy
and community follow-up for all patients from the TB clinic. The service is also responsible
for contact tracing, screening and BCG vaccination when required.
Cystic Fibrosis
Most adult patients with CF attend the regional centres in Leeds or Newcastle. A small
number of adult patients have shared care with the Respiratory Consultants.
Nursing Organisation
The two wards within the Respiratory Unit is currently managed as one by the Unit Manager.
Each of the two wards has 2 clinical sisters with an average of 4 RN’s on duty during the day
and 2 at night. There is a Clinical Matron whose responsibilities include Infection Prevention
and Control, Essence of Care, Patient Safety, monitoring and audit. There is a regular
education programme with annual career appraisals.
Management
The Division of Acute Medicine is Lead by a Chief of Service and Divisional Manager in
conjunction with the Divisions Management support team. The Directorate in managed by
the Clinical Director and Directorate Manager.

Clinical
The post includes the provision of respiratory and general medical services on behalf of
South Tees Foundation Trust from the local PCT’s (Middlesbrough and Redcar and
Cleveland) and other patients from outside these PCT’s who are referred from the Regional
Specialities working inside the Trust.
The appointee will share a 5 week rota; 2 weeks ward based with daily ward rounds and
ward procedures and 3 weeks outpatient clinics and procedures. The on-call rota is one full
week every 5 weeks. One/two peripheral clinics a month are included in this post.
On-occasion some cross cover with the FHN may be required in order to support services
and facilitate integration.
Teaching
Undergraduate and post-graduate teaching forms an integral part of the post with a
commitment to medical student teaching in the ward environment and both ad hoc and
formal teaching sessions for junior doctors. Regular MRCP PACES Programmes are hled
with Regional Mock Examinations. JCUH has hosted MRCPn Part 11 exams. Medical
students from Newcastle Medical School are regularly attached to all of the medical units
and all Consultants are asked to share in both structured and bedside teaching.
Research and Audit
Strong links exist with the audit department and the appointee will be expected to have some
direct involvement in audit. Research is an integral part of the Directorate and these
activities are encouraged. South Tees NHS FT has well established links with Newcastle,
Durham and Teesside Universities. The Acute Medical Division has three professorial chairs
and three senior lecturers, more academic appointments are planned. The Trust is keen to
develop clinical research and has invested in premises and a clinical trials unit to promote
this.
CPD
The successful candidate will be expected to take advantage of continuing professional
development. The Medical Director holds the budget centrally and all reasonable requests
for study leave have been honoured to date and funded within the allowances contained with
the Medical and Dental Whitley Council and its successors.
Appraisal
Annual appraisals are mandatory for all Consultants and are carried out by the Clinical
Director. Appropriate time and assistance will be provided for preparation.
Managerial Duties
The successful candidate will be expected to undertake the ordinary administrative
duties of a Consultant post. These will include participation in the process of clinical
governance, service development, business planning and related activities.
This post includes no specific managerial duties. There is an implicit expectation that the
post-holder will help to ensure that inpatient and outpatient services are managed as well as

possible and that help will be offered wherever necessary to meet reasonable service targets
and standards of quality assurance.
The appointee will be a member of the Senior Medical Staff Committee, The Division of
Acute Medicine and the Respiratory Directorate.
Hospital Infection Control
The Trust regards control of healthcare-associated infections as a high priority. The
appointed Consultant will be expected to promote high standards of infection control practise
among the junior doctors he/she supervises.
Job Plan
A formal job plan will be agreed between the appointee and their Clinical Director on behalf
of the Medical Director three months after the commencement date of the appointment.
The job plan will then be reviewed annually. It will cover all aspects of a consultant’s
professional practice including clinical work, teaching, research, education and managerial
responsibilities. It should provide a clear schedule of commitments, both internal and
external. In addition, it should include personal objectives, including details of their link to
wider service objectives and details of the support required by the consultant to fulfil the job
plan and the objectives.

MAIN CONDITIONS OF SERVICE
The terms and conditions are identical to the new Consultant Terms and Conditions of
Service of Hospital Medical and Dental Staff (England and Wales) and General Whitley
Council Conditions of Service, as amended from time to time.

Any Consultant who is unable to work full time for personal reasons will be eligible to
be considered for the post. If such a person is appointed, modifications of the job
content will be discussed on a personal basis in consultation with consultant
colleagues and the Medical Director.
2.
The successful applicant will normally be required to reside within the ‘traditional’ ten
miles (or within 30 minutes) of their base hospital although reasonable requests to
extend this are usually agreed by the Medical Director.
3.
The appointee will be subject to a medical examination.
4.
The current salary applicable to the post will be in line with the new Consultant
Contract.
5.
Relocation expenses will be offered and are negotiated on an individual basis with
the successful applicant.
6.
Secretarial assistance and office accommodation will be made available.
Travel and Subsistence
Travel and subsistence will be reimbursed for preliminary visits only to those candidates
selected for interview. Reimbursement is restricted to two such visits, whether they are
made before or after the constitution of a shortlist. In the case of candidates travelling from
abroad, travelling expenses are normally payable from the point of entry into the United
Kingdom.
