1.
Outline of the post
We are seeking to appoint a consultant radiologist with an interest in gastrointestinal
imaging/intervention to support the diagnostic imaging services in NHS Lothian delivered
through NHS Lothian Acute Services Hospitals The main duties of the post will relate to
general radiology, CT, MR and GI imaging/intervention. The post will be based at the Royal
Infirmary; the appointment will be to NHS Lothian and, as imaging services develop across
Lothian, the post holder may be required to work at other sites depending on the needs of the
service.

NHS Lothian
NHS Lothian is an integrated NHS Board in Scotland providing primary, community, mental
health and hospital services. Mr Tim Davison is Chief Executive and Dr David Farquharson is
Medical Director.
The NHS Board determines strategy, allocates resources and provides governance across
the health system. Services are delivered by Lothian University hospitals division, the Royal
Edinburgh hospital and Associated mental health services, 4 community health (and social
care) partnerships (CH(C)Ps) in City of Edinburgh, West Lothian, East Lothian and
Midlothian, and a Public Health directorate.
NHS Lothian serves a population of 850,000.
Acute Hospitals Division
The Acute Hospitals Division (previously the University Hospitals Division) provides a full
range of secondary and tertiary clinical services to the populations of Edinburgh, Midlothian,
East Lothian and West Lothian. The Division is one of the major research and teaching
centres in the United Kingdom.

The Royal Infirmary (RIE) is a major teaching hospital on a green field site in the South East
of the city of Edinburgh built in 2003. It comprises 25 wards, 900 beds, and 24 operating

theatres, and is equipped with modern theatre and critical care equipment and monitoring.
Within the main building is a dedicated, multidisciplinary, 5 theatre day surgery complex. The
hospital provides for most specialities and is the centre for:











General surgery with a focus on the upper GI tract
Vascular surgery
Hepato-biliary and Transplant medicine and surgery
Cardiac and Thoracic surgery

There is a Combined Assessment Unit which takes unselected GP or direct emergency
referrals,. This includes the Dept. of Liaison Psychiatry and the Scottish Poisons Bureau and
Treatment Centre. There are full supporting Laboratory and Diagnostic Radiology Services
(including CT, MR, Ultrasound, NM and PET/CT). There is a full range of lecture theatres, a
library and AV facilities.
The Western General Hospital (WGH) has 600 beds and 5 operating theatres and is
equipped with modern theatre and critical care equipment and monitoring. The Anne
Ferguson building was completed in 2001. The hospital provides for most specialties and is
the centre for:

There is an Acute Receiving Unit, which accepts GP referrals and 999 ambulance medical
cases on a zoned basis within the city, and a nurse led Minor Injuries Unit. There is no
trauma unit at this hospital. There are full supporting Laboratory and Diagnostic Radiology
Services (including CT, MR, Ultrasound and NM).There is a full range of lecture theatres, a
library and AV facilities.
The Department of Clinical Neurosciences will relocate with the RHSC to a new build hospital
at the RIE on the Little France campus in 2017.
St John’s Hospital opened in 1989 and is located in the centre of Livingston; a new town
about 30 minutes drive west from Edinburgh. The hospital provides for most common
specialties but does not have emergency general surgery or orthopaedic trauma operating.

Since 2005 general surgery and orthopaedics have been reconfigured in NHS Lothian with
SJH being developed as a major elective centre for the region. Lothian’s ENT service was
relocated to SJH to create an integrated head and neck unit with OMFS and Plastic Surgery.
Recent developments at SJH include a new endoscopy suite, an Intensive Psychiatric Care
Unit, a digital mammography unit, an oncology (cancer care) day centre, a satellite renal
dialysis unit and a £2.75m reprovision of A&E. There are full supporting Laboratory and
Diagnostic Radiology Services (including CT, Ultrasound and NM). A 1.5T MR is being
installed and due to be operational in September, 2014.
The hospital has been accredited full teaching hospital status by the University of Edinburgh.
There is a full range of lecture theatres, a library and AV facilities.
The Royal Hospital for Sick Children (RHSC) is a 151 bedded hospital providing general
and specialist services for children. The hospital is situated in a residential area close to the
centre of Edinburgh and is approximately 3 miles from the site of the New Royal Infirmary and
the co-located University of Edinburgh Medical School and 3 miles from the Western General
Hospital. The RHSC is the main paediatric teaching hospital for the South-East of Scotland
providing general and specialised services on a local, regional and national basis. It acts as
the local paediatric referral centre for the children of Edinburgh and surrounding areas, and
as a tertiary referral centre for intensive care patients; gastroenterology, hepatology &
nutrition; respiratory medicine; cardiology; nephrology; neurology; oncology; haematology;
neonatal surgery; plastic surgery; orthopaedic surgery; urological surgery and aspects of
general surgery.
Hospital accommodation encompasses five theatres, a critical care unit comprising a 6/8
bedded Paediatric Intensive Care Unit, 4/6 bedded High Dependency Unit and a 3 bedded
Neonatal Intensive Care Unit. There is an excellent library facility and a modern lecture
theatre with a full range of audio-visual equipment.
All services are supported by comprehensive radiology, neurophysiology, laboratory and
therapy services. The local radiology department provides on site Magnetic Resonance
Imaging, CT Scanning, nuclear scanning and ultrasound. On site laboratories provide
biochemistry, haematology, pathology and neuropathology services.
There is active planning taking place to re-provide the RHSC on the RIE campus at Little
France in 2017.

population of Edinburgh, Midlothian, East Lothian and West Lothian.

The four CHPs are coterminous with Edinburgh, Midlothian, East Lothian and West Lothian

There are 7,500 members of staff. In addition, there are approximately 1,000 independent
contractors in General Medical and Dental Practice, as well as pharmacists and opticians. A
population of 850,000 people is served across health board area. The range of services care
of the elderly, medical rehabilitation, community mental health, substance misuse and
learning disability, district nursing and health visiting, family planning, well woman, ,
comprehensive dental care and those provided by Professions Allied to Medicine, such as
physiotherapy, pharmacies and optometrists. Specialist services provided include brain injury
rehabilitation, bio-engineering and prosthetics, drugs and alcohol misuse and harm reduction,
AIDS/HIV and Children and Family Psychiatric Services.
Royal Edinburgh Hospital and Associated Services
The Royal Edinburgh and Associated Services provides a range of Mental Health services to
the population of Lothian and other Boards within Scotland.
The Royal Edinburgh Hospital is located on the south side of the City of Edinburgh. It
comprises some 20 wards, 420 beds, day hospitals and outpatient facilities. The hospital

There are an additional 46 bed and 1 day hospitals for Psychiatry of Old Age in the north of
the city at the Royal Victoria Hospital.
The hospital is currently housed in a mix of accommodation ranging from 19th century to
present. There is a major project now in place to take forward a reprovisioning programme in
line with the strategic vision with the “Delivery for Mental Health” Scottish Executive 2006.
Department of Public Health Medicine
The aim of the Department is to improve the health of the people of Lothian in collaboration
with many other partners. Using our range of knowledge, experience and networking
capability, our distinctive contributions are the promotion of specific measures to monitor and
improve health and the collation and interpretation of health related information.
To enable efficient management of the Department: there are at present four groups in the
Directorate. These are; Healthy Communities, Healthcare; Health Protection and Health
Information.

University of Edinburgh
The University of Edinburgh was established in 1582 and is one of the largest in the United
Kingdom; located on many prominent sites in Scotland’s capital city. It is Scotland’s premier
research university, graded within the top six multi-faculty British Universities in the last
national research assessment exercise (90 percent of its academic staff were in units rated 4,
5 or 5*). It has 3,000 academic staff, over 16,000 undergraduate and over 4,000
postgraduate students and an annual expenditure of over £261M for teaching and research.
The University is organised into 3 Colleges: Humanities and Social Science, Medicine and
Veterinary Medicine, Science and Engineering.
The Clinical Research Imaging Centre is located in the Queen Margaret Research Institute of
the University of Edinburgh on the Royal Infirmary campus. The CRIC is a joint
University/NHS Lothian imaging facility opened in 2009. It is a state-of-the-art centre for
human integrated imaging which includes a 320 slice CT scanner , a 3T MR system and a
CT-PET (128 slice) scanner. This is served by a dedicated cyclotron with radiopharmacy suite
(7 hot cells).

There are excellent facilities on all sites.

The Radiology Directorate
The Radiology Directorate is located within the Diagnostic Services group and is led by the
Clinical Director, Dr Paul Allan, who is managerially accountable to the Head of Service, Mr
Aris Tyrothoulakis and professionally accountable to the Associate Medical Director, Dr Brian
Cook. The Clinical Director is supported by a Service Manager, Mr Clinton Heseltine; there
are Lead Radiologists for RIE, WGH/StJH and RHSC. The Radiology Directorate provides
imaging services for each of the component hospitals within the Acute Hospitals Division, as
well as for Primary Care Services.
There is a Lothian-wide Hospital Information System which is being developed into a full
Electronic Patient Record. The associated PACS system has been installed as part of the
national PACS project, which allows review of images from all radiology departments across
Scotland and has been updated in 2014. The local PACS network allows flexibility in
appointing patient examinations to the different departments and subsequent reporting of
these examinations by the relevant radiologists.

The Department of Radiology, Royal Infirmary
The Department of Clinical Radiology at the Royal Infirmary is within the Radiology
Directorate of NHS Lothian. The Department offers a wide range of imaging and
interventional services. The Clinical Lead within the Department is Dr Fergus Perks and the
Associate Directorate Manager for the department is Stephen Evans. There are satellite
radiographic departments for the Accident and Emergency Department and the Orthopaedic
Outpatient Department. A further satellite imaging department is sited in the Simpson Centre
for Reproductive Health.

Primary Care Imaging Service
The Primary Care Imaging Service is delivered on three main sites: Leith Community
Treatment Centre in north Edinburgh; the Lauriston Building in central Edinburgh; and
Roodlands Hospital at Haddington, East Lothian. There are 12 WTE
radiographers/sonographers. The work is predominantly plain film reporting and ultrasound.
In 2013-14, PCI carried out 46037 examinations: 28861 plain films; 11414 general
ultrasounds and 4438 Ob/Gyn ultrasounds. The service is now delivered by radiologists at
RIE and WGH/StJH.
7.
Details of the Post
We are seeking to appoint a consultant radiologist with an interest in GI imaging/intervention
to contribute to the imaging service at the Royal Infirmary. The main duties of the post will
relate to general radiology (including primary care reports), US, CT, MR and GI
reporting/intervention. There are a number of multidisciplinary team meetings during the week
and allowance to attend those meetings relevant to an individual’s practice is made in job
plans.
The post will be based at the Royal Infirmary. Time may be allocated to other departments in
Lothian, depending on the outcomes from a review of Radiology Services. Both the Royal
Infirmary and Western General have responsibility for a significant proportion of the work
done in primary care imaging and the new appointment is expected to contribute to this work.
Significant developments are also taking place throughout the imaging service as we move to
Single System Working across Lothian and all consultants will be expected to be flexible with
their working patterns.
The consultant will participate in teaching of undergraduates and specialist registrars,
management tasks and audit. Consultant job plans are reviewed annually by the Clinical
Director or Deputy and adjusted subject to the needs of the Department and the individual by
mutual agreement. Electronic job planning is being introduced for consultants across Lothian
in 2014-15. A programme of annual appraisal is in place.
Shared office accommodation and secretarial support will be provided.
A provisional job plan is given in Appendix 1. NHS Lothian allocates all consultants one SPA
for CPD, audit, clinical governance, appraisal, revalidation, job planning and routine internal
communication and management meetings. As a major teaching and research contributor
NHS Lothian would normally expect to allocate additional SPA time for activities to do with
undergraduate education, educational supervision of trainee medical staff, research, service
development and other activities. These are all areas where NHS Lothian has a strong
commitment and we recognise the contribution that consultants are both willing and eager to
make. The precise allocation of SPA time and associated objectives will be agreed with the
successful applicant.
The department at the Royal Infirmary undertakes weekend working with appropriate time off
the following week included in the job plan. Extended day working is being explored and will
be instituted towards the end of 2014.
On Call: This will be carried out on the RIE General Rota, currently 1:15. (Level 1, 3%
availability supplement). There is a separate interventional rota at RIE, which is currently 1:6.
Predictable on-call worked is allowed for in job plans. However, with the installation of PACS
across Lothian, on call arrangements across the city are being reviewed.

Research and Development
The Directorate is keen to develop strong links with various university departments as well as
academic links with other NHS groups. There are opportunities for collaborative research
with these departments, many of which have strong and established research programmes.
The locum consultant will be expected to participate in teaching and research.

Teaching
SpR training is co-ordinated by the Radiology Postgraduate Training Committee. This is
chaired by the Training Programme Director for the South East Scotland Radiology Training
Scheme, Dr J Anderson. It includes the Regional Postgraduate Adviser, Dr D Summers, the
Educational Supervisors, two junior staff representatives and the Postgraduate Dean.
Consultants are also expected to contribute to the teaching of medical students at various
stages of their training.
