Student is entering with special education records, including current IEP.
□ School and parent(s) decide the IEP addresses identified student needs and receiving school determines that current IEP can be
implemented as written (goals, progress, special education and related services).
□ Provide FAPE to the student, including comparable services to those on existing IEP.
□ Open an “In Process” IEPPro demographics page, update access list, and notify related servers within 3 school days.
□ Conduct a data review to determine if an initial evaluation is necessary to ensure the student meets the Utah and LEA eligibility
requirements. (Team may review existing data to determine what, if any additional assessments are needed to determine eligibility.)
 If LEA determines new evaluation is necessary, obtain a permission to test and follow initial evaluation and eligibility
determination procedures. You will have 45 school days to complete this evaluation and hold an eligibility meeting.
 If LEA determines new evaluation is NOT necessary, then complete an eligibility form from IEPPro. Mark “Consider
eligibility” on the IEP Notice of Meeting that is sent out for the IEP meeting noted below. Complete eligibility form at that
meeting. (Note: Ensure that eligibility determination criteria comply with Utah requirements).
□ Gather present levels data, and invite parents/ guardian and related servers to an IEP meeting within 15 school days –
putting your PLAAFP, goals and services on IEPPro. The next IEP will not be required for 364 days.

□ School and/or parent(s) decide the IEP does not address identified student needs and receiving school determines that
current IEP cannot be implemented as written (goals, progress, special education and related services).
□ Provide a FAPE to the student, including comparable services to those on existing IEP.
□ Provide Notice of Meeting and hold an Interim IEP within 10 school days, written on ASD IEPPro (with related servers notified).
□ Conduct a data review to determine if an initial evaluation is necessary to ensure the student meets the Utah and LEA eligibility
requirements. (Team may review existing data to determine what, if any additional assessments are needed to determine eligibility.)
 If LEA determines new evaluation is necessary, obtain a permission to test and follow initial evaluation and eligibility
determination procedures. Please complete the testing and hold eligibility meeting with the 30 day IEP meeting below.
 If LEA determines new evaluation is NOT necessary, then complete an eligibility form from IEPPro. Mark “Consider
eligibility” on the IEP Notice of Meeting that is sent out for the interim IEP meeting noted below. Complete eligibility form at
that meeting. (Note: Ensure that eligibility determination criteria comply with Utah requirements).
□ Gather present levels data, provide Notice of Meeting, develop a new IEP and hold IEP meeting within 30 days after Interim IEP.

Receiving student special education records.
□ Upon receipt of special education records, review file and ensure special education file contains at least minimum requirements:
 Signed parental Consent for Initial Placement
 Current Eligibility Determination with an Evaluation Summary Report
 Current IEP
□ Have team decide what additional data/documentation are needed to correct incomplete or incorrect file contents. Take necessary
actions to ensure file compliance.
Note: Experiencing difficulty in obtaining the IEP from the previous LEA does not relieve the current LEA of its obligation to have a
current IEP in place for an eligible student.
