1) When completing the Notice of Eligibility Committee and/or IEP Team Meeting
form, I document all attempts (at least 3) to reschedule the meeting to
accommodate the parent, and attempts to get the parent to respond to the notice.

2) If the parent indicates he/she cannot attend, I offer alternative ways to
participate in the meeting (e.g. video or telephone conferencing), and place an *
beside any options I ask the parent to consider.

3) If the parent’s agreement with the excusals has not been documented on the
Notice of Eligibility Committee and/or IEP Team Meeting form, then I make sure the
Team Member Excusal(s) form is completed to confirm the parent’s agreement.

4) If a team member’s area is to be discussed at the meeting, and they will not be
present, I ensure the team member has prepared the In Lieu of IEP Team
Attendance Report for the parent’s review prior to the development of the IEP.

5) If both the parents and the district agree to amend the student’s current IEP
rather than have an IEP meeting, I ensure the parents are given prior written notice
informing the parent of what has occurred in this amendment and how it will
change the student’s services and/or placement.

6) When completing the Notice of Individual Evaluation/Reevaluation Request form
I ensure the parental consent is obtained and documented before any evaluations
are conducted, and the parent knows they have the right to request an assessment.

evaluation, I enclose the Procedural Safeguards brochure.

8) I ensure whomever receives the Notice of Individual Evaluation/Reevaluation
Request form when returned by the parents documents the date the form is
received, because it is the date that begins the timeline for completion of the
evaluation(s) (i.e., initial - 80 calendar days; additional - 60 calendar days).

9) When completing the Eligibility Determination Checklist, if the eligibility
committee is considering a student for eligibility as a student with a SLD, I make
sure we also complete the Specific Learning Disabilities Team Report.
10) When completing the Prior Written Notice form, I make sure it is written clearly
so the parent is able to clearly understand and articulate what is going to happen
for his or her child and is completed within 5 days of the proposed or refused
action.
