To work as an integral part of a multidisciplinary team To provide clinical mental health assessments and time limited direct treatment service to consumers across the life cycle. To build capacity in the local community in responding to high and low prevalence disorders. To assist the Clozapine Coordinator in providing a service to teams within Mercy Mental health and GP’s participating in Clozapine Shared Care. To participate in the development of research activities where appropriate. This role is offered on a full time basis. Qualifications: The successful applicant will have the following qualifications: Registered as a Nurse with the Nurses Board of Victoria in Division 1 with approved post graduate qualifications in Psychiatric Nursing or registered as a Psychiatric Nurse with the Nurses Board in Division 3; A current Victorian driver’s license is essential to the role Resource management: Total staff management (FTE): Annual Operating Expenditure: Annual Capital Expenditure: N/A N/A N/A Personal Competencies required Sound written and verbal communication. Commitment to the delivery of services that are responsive to clients’ cultures, gender and family issues Standard Measures Mental health assessments are undertaken at the request of the primary health care provider and in collaboration with this person Care plans are discussed and authorised by the clinical team MHAC Key Activities Ensure that all primary health care providers and consumers are followed up in a timely manner Ensure that the needs and strengths of consumers are assessed and that treatment plans are responsive to the needs of the consumer and communicated to the primary health care provider. Any direct service provided is to be time-limited and indicated by clinical evidence. Is available to participate in other Registered Nurse Grade 3 – Shared Care Programs Key Result Areas Ensures that consumer care is appropriately documented. Standard Measures All consumer interventions, care planning and reviews are recorded in the consumer file on the relevant form and are updated according to agreed timelines All RAPID contacts are documented/entered by the deadline each month Contributes to the provision of optimal care through effective liaison, consultation and collaboration with internal and external service providers. MHAC Staff member will liaise with all relevant others involved in providing care to mutual consumers and will document these contacts and collaborative decisions Collaborate with primary health care providers involved in the consumer’s care. Provide clinical consultation and advice in response to requests from primary health care providers and document these contacts. Provides information and advice to other components of the mental health service around high and low prevalence disorders and engagement with primary health Key Activities activities to support the delivery of program functions Routinely records all assessments, care plans, reviews and consumer interventions in the appropriate manner in the consumer file Review care plans within agreed timeframes Outcome measures completed as required Documents all relevant contacts appropriately and in a timely manner Initiates contact with relevant others involved in care Consult and facilitate interventions that encourage early intervention and crisis prevention approaches to consumer care Document contacts with other agencies in the consumer file Adopts a partnership approach in working with internal and external stakeholders.
