Note: Not all Red Cross Shelters accept pets 2. __ My pets are current on their yearly Veterinary Exam and Vaccines 3. __ I have a copy of their Vaccination Records 4. __ My pets have a Microchip implant and registration for identification purposes, should we become separated 5. __ My pets have a tag on their collar that has contact information and any medical alert information, should we get separated 6. __ I have a safe way to transport and house my pets: Pet Carrier or collapsible pen (baby pen, wire cage) 7. __ I have a Litter Pan and Kitty Litter 8. __ I have a Collar or Harness and a Leash for each pet 9. __ I have at least a 7-day supply of Food and Water (Bottled Gallons) for my pets 10. __ I have Food and Water Bowls 11. __ I have at least a two-week supply of medications that my pet needs including sedatives and motion sickness medication for travel 12. __ I have Bedding materials: Blankets, Towels, Pet Bed 1 13. __ I have cleaning supplies: Garbage bags, towels, Simply Green etc. 14. __ I have a full tank of gas and have checked my vehicles oil and tires 15. __ I have Flashlights, a Radio, Batteries, Duct Tape, Phone and Charger, something to write with, Notepad etc. 16. __ I have Phone Numbers for pet friendly motels, Veterinarians and Boarding Facilities in the city of my destination: www.petfriendly.com, www.dogfriendly.com 17. __ I have Phone Numbers of friends and relatives. Horse Specific Items 1. __ My horses are current on their Veterinary Exam and Vaccines 2. __ I have a copy of their Vaccine Records 3. __ All of my horse have a current Yearly Coggins Test and I have a copy of the Coggins Test for each horse: Required by Georgia Law 4. __ I have a 30-day Health Certificate for each horse from my veterinarian for travel in the state of Georgia and for travel outside of the state: Required by Georgia law to transport horses or other livestock 5. __ I have flashlights, radio, phone and charger, Duct Tape, something to write with and a note pad 6. __ I have a Feed and Water Buckets and straps to secure the buckets 7. __ I have at least a one-week supply of feed and hay 8. __ I have an adequate supply (2-3 days) of transportable water 9. __ I have fans and straps to secure the fans to a stall for keeping my horse cool once I reach my destination 10. __ I have a portable generator (maybe) 11. __ I have cleaning supplies: Wheelbarrow, poop scoops-pitch forks etc. 12. __ I have halters and lead ropes that are in good repair 13. __My horses are trained to load onto and off of the horse trailer and trained to trailer safely 14. __ I have made transportation arrangements if I don’t have a horse trailer or am not able to transport all of my horses in my horse trailer: Professional Transport Companies, friends etc. 15. _ I have checked my horse trailer: Tires are in good condition with the proper amount of air, and I have a usable spare tire, jack, lug nut wrench, WD 40, the lights work, the brakes work, the bearings and axels are in good repair, and the floor of the trailer is in good shape 16. __ I have reservations at a boarding facility in the city I am evacuating to: possibly The Georgia National Fairgrounds in Perry Georgia, www.gnfa.com, 478-987-3247 17. __ I have identification tags for the horses halters with contact information on the tags 18.__My horses have a Microchip Identification and registration: This can be performed by your Veterinarian 2
