POSITION DESCRIPTION:
BASIC PURPOSE:
Under general supervision of the med support program manager, provides a variety of professional child
and adolescent psychiatric/mental health nursing and patient care services to support the functions and
operation of the Youth for Change Medication Support Program.
Distinguishing Characteristics: This position requires a California state Licensed Vocational Nurse or
Registered Nurse and a minimum of four (4) years of nursing experience; reports to Med Support RN
Program Manager.
WORKING RELATIONSHIPS:
Internal Relationships: Med-Support Staff, Collateral YFC Staff and Clients.
External Relationships: Collateral Medical Care Providers and Human Services Providers.
ESSENTIAL DUTIES AND RESPONSIBILITIES:










Interviews and observes patients to ascertain medical/psychiatric history and current condition;
reviews patients’ records; takes vital signs, assesses current condition, and records and reports all
information, including medication history and compliance.
Administers general nursing care to patients in a clinic or community setting; performs nursing
assessments; observes patients' conditions and behavior; reports significant changes to supervisor
and physician and records nursing notes on patient chart; gives first aid as needed.
Assembles nursing supplies, prepares patients for evaluation, and assists in evaluation and
treatment planning for patient.
Provides direct patient assessment per written clinic protocols, policies, and procedures.
Engages in the telepsychiatry process for the delivery of psychiatric care and education, as well as
“in-house” psychiatric services.
Counsels patients, giving supportive care to patients and family.
Reads and evaluates written information such as medical/psychiatric reports, physicians’
evaluations and diagnosis, and staff notes; evaluates problem presented by clients and
develops treatment plans in collaboration with physician and staff.
Instruct individual patients on follow-up care, birth control, communicable disease prevention,
hygiene, general health care, treatment of specific medical and/or psychiatric conditions and
Illness and Injury Prevention.
Provides assistance with maintenance of records, inventory and physical security of drugs and
medical supplies; orders and requests supplies as needed.
Prepares records and reports as needed for program development.

Coordinates referrals for health testing; works closely with public health nurses on communicable
disease control and treatment.
Coordinates referrals for pediatric specialists; cardiology, neurology, endocrinology, etc.
Performs administrative duties; receives, logs in and files a variety of reports, client charts and
other documents; responds to and refers incoming telephone calls.
Prepares room and equipment for use by Psychiatrist; assures needed charts and chart forms are
readily available.
Provide medical case management for eligible clients per specific program guidelines
Provides client centered education and counseling services regarding a variety of health topics per
department protocols, with major focus on psychotropic medication therapy.
Engage in training, technical assistance and quality assurance activities with county, public and
private health care providers; schools and community agencies per specific program guidelines.
Participate in program development, analysis, and outreach.
Assists Youth for Change staff and volunteers as appropriate.
Performs other related duties as assigned.

TRANSPORTATION:
If use of your personal vehicle is necessary for Youth for Change business, you will be
responsible for keeping a mileage record and submitting it to your supervisor for reimbursement.
Proof of current liability insurance on your personal vehicle must be provided at time of
employment and when it expires. Personal vehicle insurance is the primary coverage in case you
are in an accident in your vehicle while on business for YFC. Since you will be transporting
children in your vehicle, it must be well maintained.
MINIMUM QUALIFICATIONS
Required Knowledge and Skills:
 Knowledge of modern nursing principles, techniques, and procedures for the care of children and
adolescents.
 Knowledge of medical terminology and the use of medical equipment and supplies.
 Knowledge of medicines, particularly psychiatric medicines and their effect on the patient.
 Knowledge of Agency’s policies and procedures.
 Knowledge of county, state and federal statutes, rules, ordinances, codes and regulations.
 Knowledge of the principles and practices of medical file and records management, as well as
documentation procedures.
 Skill in providing effective nursing services, administering medications, assessing difficult situations
and taking effective courses of action.
 Skill in evaluating mental health problems, and making recommendations based on observations.
 Skill in interacting with people of different social, economic, and ethnic backgrounds.
 Skill in assessing and prioritizing multiple tasks, projects and demands.
 Skill in establishing and maintaining effective working relations with co-workers, other Youth for
Change employees, representatives from other governmental agencies and the public.
 Skill in operating a personal computer utilizing a variety of business software.

Education, Experience, Certifications and Licenses:
 College degree and four (4) years of professional experience as a nurse preferred.
 Psychiatric/Mental Health experience is highly preferred.
 Must be licensed as a Vocational or Registered Nurse from the State of California, and remain active
with all annual licensing requirements.
 A valid State of California driver's license is required.
LANGUAGE ABILITY:
Ability to document encounters and progress notes accurately. Ability to effectively present
information and respond to questions from co-workers, managers, clients, and general public on a
need to know basis. Ability to read and comprehend simple instructions, short correspondence,
and memos. Ability to write simple correspondence. Ability to effectively present information in
one-on-one and in small group situations to co-worker, vendors, and employees of the
organization.
MATHEMATICAL SKILLS:
Ability to add, subtract, multiply, and divide.
REASONING ABILITY:
Ability to apply common sense understanding to carry out instructions furnished in written, oral,
or diagram form. Ability to deal with problems involving a few concrete variables in
standardized situations.

Duties are performed inside with climate control.
This position regularly requires being able to work at close vision for the purpose charting and recording
keeping and distance for home visits.
The typical noise level in the work environment of this position is of moderate notice. On occasion, you
may be subjected to a loud noise level.
WORK ENVIRONMENT – SOCIAL
Employee shall work in a non-hostile work environment. That is, the employee is expected not to
engage in any activity that unreasonably interferes with performance of any other employee, such
