Student Perception Survey
Evaluations must be completed with #2 pencils.
1. Evaluations are to be completed during the last 5 minutes of the regularly scheduled class time on
Thursday, March 22, 2012. The department secretary will pass out the surveys and give directions to
students for filling out the evaluation.
2. STUDENTS – PLEASE WRITE THE FOLLOWING INFORMATION AT THE TOP OF THE
EVALUATION FORM. – Each sheet must include instructor name, course number, and section
number.

Important: The instructor will not be present when surveys are completed by students.
3. Evaluation form WILL NOT SCAN IF:
 Pencil marks extend too far beyond the given area
 Forms are creased, torn or bent
 Students have marked too lightly
4. Also, students may include additional comments on the back side of the survey form.
5. The department secretary will collect the completed surveys.
NOTE: Instructors will not see the student evaluation results and comments until after the
semester grades are posted. Instructors will not read the actual handwriting of students.

will not see the results until after the semester is complete and final grades are submitted.
