Cincinnati Children’s Hospital Medical Center is conducting a search for a Pediatric Patient/Family Centered Medical Home Learning Community (PCMH) Coach to assist practices through their journey of becoming National Committee of Quality Assurance (NCQA) recognized. This position will report to the Manager of PCMH. Cincinnati Children’s formally launched its Pediatric Patient/Family Centered Medical Home Learning Community in late 2013, focusing on: Improving access and continuity of healthcare for children in our community through stronger partnerships between patients, families and care teams; and Facilitating support for physician practices on the PCMH journey and through transformation, including development of reliable and measurable processes of care The Pediatric Patient/Family Centered Medical Home Learning Community includes both internal and external stakeholders. The goal of the Learning Community is to maximize current and future opportunities which result from an efficient, patient/family-focused model of care. The Medical Director and PCMH Coaches assist CCHMC primary care and specialty practices, as well as community pediatric primary care practices, through their transformation and journey of becoming NCQA recognized. PCMH Coaches are responsible for evaluating assigned practices relative to their readiness for transformation and identify gaps relative to the practice’s ability to meet NCQA standards. PCMH Coaches create, execute and evaluate the tools, processes and templates used by the practice, make recommendations for improvement and enhancements with the goal of creating “best in class” submission documentation. During the transformation journey, the PCMH Coaches provide ongoing onsite and/or telephonic coaching and team consultation sessions with the practice staff. PCMH Coaches assist in developing educational materials and courses to promote understanding and acceptance of the patient centered model. PCMH Coaches are responsible for planning and leading educational programs and special projects within the PCMH program. Ideal candidates will have a Bachelor's Degree with 5 years of related experience in development and implementation of PCMH models and knowledge of NCQA Recognition Program.
