Use data effectively to guide & monitor our work •Make sure data’s available, useful, and understood by all. •Help teams use data to drive change. • Help PCMH teams & Coach circumvent implementation barriers. Project manage to ensure momentum is maintained and we stay on track. • Facilitate consistent & effective team meetings. Guide PCMH teams & Coaches. • Help solve problems/barriers. • Gradually build self-sufficiency within each site’s PCMH team.
