Early/Middle College High Schools or Programs District: Count Date: Building/Program: This form is for identifying pupils enrolled in an early college or middle college high school or program established by an agreement between an ISD or school district and a public or private college, university, community college or junior college, and recognized as a formal E/MC high school or program by the Michigan Department of Education. These 5 year public high schools/programs are designed to allow a pupil to earn a high school diploma while also earning an associate’s degree, advanced certificate, or up to 60 transferable college credits toward a four year degree. Important: Print and attach a list of pupils included in district’s membership count who are enrolled in an early college/middle college high school program. Ensure these students are reported in MSDS with Program Participation Code = 3500.
