Parents play a key role in the IEP planning process and their child’s success in school. Because of
this, it is my goal to make the IEP planning process focused on the student and family. I have
decided to include the students in their IEPs this year with 5th grade student-led IEPs. As part of
the process, I would like to evaluate how you feel about the IEP process before and after the 5 th
grade student-led IEPs, therefore I will have you complete this survey now and then again after
the IEP in February. Your responses to this survey are strictly for my benefit to reflect and make
adjustments to my professional decision-making and planning for IEPs. I appreciate your input.
