Agreement and Signature
Pet Awareness & Welfare Services (PAWS) will instruct clients to call for an appointment. Additional
fees for services that may arise during the operation, other than those described above, will be
discussed with the client prior to surgery. All fees are to be paid directly to the veterinary practice at
time of service and the practice is responsible for collecting such fee. Pet Awareness & Welfare
Services (PAWS) agrees to pay the veterinary practice the difference of the client’s fee and the
special prices listed above.
I have a right to discontinue my participation in the program at any time by notifying Pet Awareness
& Welfare Services (PAWS).
