Maternal Outpatient Treatment Service Provider III Job Description SUPERVISED BY: Clinical Director SUPERVISES: N/A SUMMARY: Provide counseling and case management services to First Step Home’s maternal outpatient clients and those living in transitional housing at 2217 Fulton, 2221 Fulton Avenue, 925 Nassau, 955 Nassau and other locations, as needed. DUTIES AND RESPONSIBILITES 1. Provides individual counseling, group counseling, case management and crisis intervention to clients using the Maternal Addictions curriculum, as established. Work with the client to develop an Individual Treatment Plan according to OMHAS and CARF person/family-centered standards. Review and monitor services and activities, documenting progress according to the descriptive plan of the Maternal Addictions Program. Attend individual supervision session with the Clinical Director. Provide updated assessment within 24 hours of entry in the Maternal Outpatient Program. Develop transitional planning document for each client prior to exit. Conduct BH Mod Survey when the client exits the Maternal Outpatient Program; forward survey to recordkeeping. Provide referral to community based services as identified prior to discharge. Continue to assess assigned clients while they are in Continuing Care. Provide referrals and monitor attendance at self-help groups for assigned clients. Responsible for providing accurate and timely reports to all agencies with a legitimate interest in the progress of Continuing Care clients who have been assigned. Attend clinical meetings, assisting in deciding residential maternal clients’ readiness to be in a less structured level of care and reporting on the progress of maternal outpatient and maternal Continuing Care clients. Participate in the Clinical Team’s On-Call rotation schedule. Through Case Management, Individuals, Crisis Intervention, Groups, and other billable services generate direct billable service hours according to OMHAS productivity standards. Responsible for submitting all documents relevant to services rendered according to First Step Home, Inc. standards. Work to attain the goals as written in job performance evaluations. Abide by the standards as set forth in First Step Home’s Compliance Plan. Responsible for following all safety rules and safe health practices of First Step Home, Inc. Pursue training necessary for enhancing clinical skills and acquiring and maintaining appropriate credentials and licenses. Comply with all agency policies, procedures and directives of immediate supervisor, the President/CEO and the Board of Trustees of First Step Home, Inc.
