LIMA CITY SCHOOL DISTRICT JOB DESCRIPTION Title: SPECIAL SERVICES CASE MANAGER File 125 Reports to: Director of Special Education Job Objective: Provide psycho-educational academic and behavioral assessments, program planning and consultation services that are consistent with the district’s mission and mandated requirements. Minimum Qualifications: BS/BA degree in education and/or psychology. Meets all mandated health requirements (e.g., a negative tuberculosis test, etc.). A record free of criminal violations that would prohibit public school employment. Complies with a drug free workplace rules and board policies. Keep abreast with advances in technology and workplace innovations that enhance productivity. Ability to facilitate student/family access to appropriate community resources. Essential Functions: The following are typical work responsibilities. A reasonable accommodation may be made to enable a qualified individual with a disability to perform essential functions. Monitors education laws, rules, and regulations. Helps ensure that district policies/procedures support non-biased assessment/planning activities. Recommends procedures that facilitate compliance with legal mandates. Provide consultation services with staff, teachers, administrators, and families. Help the family/student understand how service relates to the educational program. Participate in a differentiated referral system that allows staff and parent to request an Evaluation Team Report (ETR) and/or consultation for non-disabled students. Develops a service schedule. Administer academic, behavior, and social/emotional assessments. Interpret and report assessment results. Develops and implements protocols to safeguard student assessment results that are shared with staff and/or referral sources. Ensure that confidential materials are stored appropriately. Promote effective communications and assistance. Uses problem solving techniques to tactfully address questions/concerns. Upholds board policies and follows administrative guidelines/procedures. Refers questions about district policies to administrator/supervisors. Advocates for students. Initiates referrals to community resources as needed. Complies with federal and state policies/procedures for the education of students identified as having a disability. Utilizes diagnostic instruments and techniques appropriate for the area of disability or suspected disability as part of the Evaluation Team Report process. SPECIAL SERVICES CASE MANAGER Page 2 of 3 Prepare and facilitate Evaluation Team Reports as needed. Meet Ohio Operating Standards regarding state deadlines. Consult with intervention assistance teams to help develop differentiated instructional approaches to assist children identified as At-Risk. Conduct student observations in the general education environment. Reevaluates on a three-year cycle those students having been identified and receiving special education services. Facilitate parental involvement. Consult with parents as needed (phone calls, notes, meetings, etc.). Encourage self-reliance, problem-solving, critical-thinking, creativity, and performance skills. Collaborate with other staff. Uphold student conduct code. Promote proper care and use of school property. Consult with Administration and staff. Maintain accurate records and submit reports in a timely manner. Assist in gathering and analyzing data for district purposes. Respect personal privacy. Maintain the confidentiality of privileged information. Reports suspected child abuse and/or neglect to civil authorities as required by law. Pursue growth opportunities for professional development and district goals Strives to develop rapport and serves as a positive role models for others Help students understand and embrace ethical conduct and democratic values Provides prompt notification of delays or absences.
