Adolescent Champion Health Center Manager (ACM)
Position Description
The ACM will:
1. Assist the Adolescent Health Initiative (AHI) staff in gathering clinic data, including staff, provider, and
adolescent patient surveys at baseline and year end.
2. Meet as needed with the AHI team and/or AC Provider to plan the roll out of the AC program at your site.
3. Assist in coordinating set-up of adolescent risk assessment screening and integrate into clinic flow.
4. Participate in the Adolescent Centered Environment (ACE) Clinic Assessment tool, with the Adolescent
Champion Provider (ACP), on a quarterly basis.
a. Assist in creating action plans for making clinical changes between assessments.
b. Make relevant practice/policy changes as determined by the health center team.
c. Actively communicate with ACP and AHI staff to best implement changes.
5. Participate in Adolescent Champion Team training: Three two-hour training sessions on Organizational
Change; Facilitating Staff Sparks; and Best Practices in Adolescent Centered Care; participate in one-hour
orientation and 1-2 follow-up meetings.
6. Assist ACP in coordinating and facilitating Staff Sparks at monthly all-staff meetings, and additional Staff
Sparks as determined by the AC Team (ACM and ACP).
7. Post and/or share weekly prompts that align with Staff Sparks topic following the regular Staff Sparks
presentations. Weekly prompts will be created by AHI, and delivered to ACMs ahead of time with clear
instructions.
8. Orient new staff to adolescent friendly best practices with the New Staff Orientation, provided by AHI.
Estimated time commitment is an average of 2 hours per month.
Adolescent Champion Provider (ACP)
Position Description
The ACP will:
1. Assist the Adolescent Health Initiative (AHI) staff in gathering clinic data, including staff, provider, and
adolescent patient surveys at baseline and year end.
2. Meet as needed with the AHI team and/or AC Manager to plan the roll out of the AC program at your site.
3. Assist in setting up and integrating validated risk assessment screening tool into clinic flow.
a. Educate and create buy-in among staff and providers regarding purpose and importance of risk
assessment for adolescent patients.
4. Utilize validated risk assessment screening tool for at least 70% of your adolescent patient panel.
5. Participate in the Adolescent Centered Environment (ACE) Clinic Assessment tool, with your Adolescent
Champion Health Center Manager (ACM), on a quarterly basis.
a. Create action plans for making changes between assessments.

6. Participate in Adolescent Champion Team trainings: three two-hour training sessions on Organizational

7. Facilitate Staff Sparks at five monthly all-staff meetings, and additional Staff Sparks as determined by AC
Team (ACM and ACP).
8. Complete 20 hours of adolescent-related CME throughout the year to enhance adolescent medicine
knowledge and practice.
9. Provide training for other clinic providers on adolescent friendly best practices as needed.
