Tulsa Achieves Program Agreement Form
Class of 2012
The Tulsa Achieves Program seeks to better the community by making a college education
affordable and attainable for all Tulsa County residents. The Tulsa Achieves Program will ensure that
all eligible, direct-from-high school students receive the necessary funding to pay for tuition and fees
regardless of their ability to pay.
The Tulsa Achieves Program is a gap-funding program that will supplement Federal financial aid,
State of Oklahoma aid, or other gift-aid to pay the amount of tuition and fees as eligible.
STEPS REQUIRED FOR APPLYING TO TULSA ACHIEVES
Submit on or before April 30th, 2012

2. Tulsa Achieves Program Agreement Form – Submit in person to Enrollment Services, Any Campus
 All 3 pages of this document, must be submitted to Enrollment Services on any TCC Campus
 In order to receive a signed receipt as proof of submission, this document must be submitted
in person. TCC is not responsible for lost, missing or mailed items.
3. FAFSA- Successful completion of eligible application- Submit Online
 To be successfully completed, all FAFSA documents, including any additional verification
requests, must be received by TCC’s Financial Aid Department.
Complete on or before July 16th, 2012
1. ACT/ or college placement test scores- Submit to Enrollment Services, any campus
2. Final, Official 8 semester High School transcript reflecting a 2.0 or greater G.P.A. and graduation
date – Submit to Enrollment Services, any campus.
3. Attend one TA101 session during the application period – See TA Webpage for dates/registration.
4. Meet with a TCC Academic Advisor – Go to Academic Advisement on any campus.
5. Enroll in a minimum of 3 credit hours for the Fall 2012 semester – Enrollment completed online.
APPLICATION PROCESS AND COMMUNICATION
Applicants are not officially admitted into the Tulsa Achieves Program until all requirements listed
above have been fulfilled. Throughout the application process, communication from Tulsa Achieves,
including further verification requests and acceptance notifications, will be sent to the email address
provided on the applicant’s TCC Application for Admission. Applicants may be asked to provide
additional documentation in support of their application to Tulsa Achieves, including, but not limited
to, verification of Tulsa County residency. Such notification will be sent to applicant’s email address.
GAINING AND MAINTAINING ELIGIBILITY
The Applicant must be admitted to TCC as an incoming freshmen (“direct from high school” or
“entering the first fall term after high school graduation”), graduation from high school occurring on
or before by June 2012. Once admitted to Tulsa Achieves, the student must complete a minimum of
three (3) credit hours with a passing grade each semester and must complete at least 70% of
cumulative enrolled hours with good academic standing (as defined in the TCC catalog). The student
must be continuously enrolled throughout a 16 week semester. The student must be enrolled each
fall and spring semester, with summer enrollment optional. All Tulsa Achieves students have a
registration hold that prevents enrollment each semester. Students must first seek academic
advisement in order to have the hold lifted to allow enrollment. If the student does not continue to
meet the minimum hour or academic standing requirements, the student will be placed on probation
and will remain so until the student establishes good academic standing, is suspended or attains
satisfactory progress toward a 70% completion requirement. No student may violate the Student
Code of Conduct, other departmental conduct agreements, or any College policies.
Tulsa County Residency
To be eligible for Tulsa Achieves, the applicant must maintain Tulsa County residency for at least
one year of high school and maintain Tulsa County residency while in the Tulsa Achieves program.
A student residing in Tulsa County for one year of high school will receive 25% gap funding, two
years residency will receive 50% gap funding and 3 years residency will receive 75% gap funding. A
student residing in Tulsa County all 4 years of high school will receive 100% gap funding.
Appeals Process for Removal from Tulsa Achieves
A student may appeal removal from Tulsa Achieves if a failure to meet program requirements was
the result of medical, military or undue hardship beyond their control. Tulsa Achieves Appeal forms
must be picked up and submitted at any campus Enrollment Services Office.
Restrictions
The Tulsa Achieves program funds up to sixty-three (63) credit hours or three (3) years, whichever
comes first. Enrollment during the summer semester following the students’ first fall and spring
semesters is at the discretion of the student, but would be included in the maximum number of
eligible hours, academic standing and completion ratio. Hours earned as a concurrent student and/or
prior to the first fall after high school graduation are excluded from the maximum number of hours
but are included in academic standing and completion ratio requirement calculations. Tulsa Achieves
does not cover the cost of books and/or supplies.
Gap Funding
TCC will use Federal and State funds, tuition waivers, other available aid and/or institutional funds to
meet a student’s cost of attendance at TCC. The Tulsa Achieves Program will fund the cost of tuition
and fees after all eligible Financial Aid (Pell grants, ACG, SEOG, OTAG, OHLAP) are applied.
Unless otherwise specified, third-party scholarships are not applied to institutional costs, resulting in
a refund to the student. Tulsa Achieves does not cover the cost of books and/or supplies.
Free Application for Federal Student Aid (FAFSA)
Tulsa Achieves students must successfully complete a Free Application for Federal Student Aid
(FAFSA) by the last day of April each academic year. TA applicants must comply with all
requests for additional information or financial aid documents from TCC’s Financial Aid
Department. Completion of the FAFSA must be done online. Assistance from TCC Financial Aid
Office staff, or at a FAFSA workshop, will be available. The amount of Tulsa Achieves funding may
vary each year depending on other aid received and enrollment status.
Volunteer Service
Tulsa Achieves students must complete a minimum of forty (40) hours of volunteer service per
academic year begun as an eligible Tulsa Achieves student. Volunteer service must meet the Tulsa
Achieves Volunteer Requirements provided on the Tulsa Achieves webpage. If the minimum
required service hours are not completed and reported one week after final exams of the Spring
Semester, a transcript hold will be placed on the student’s account.
Student Life Programs and Participation in College Events
Tulsa Achieves students must enroll in, and successfully complete, either Strategies for Academic
Success (ENGL 1003) or College Survival (ENGL 0963) by the end of their first spring semester.
Tulsa Achieves students must participate in academic advising, orientation sessions, and other
student life activities. Throughout the application process, correspondence from Tulsa Achieves will
be sent to the email address provided on applicants’ Application for Admission. Once admitted to
Tulsa Achieves, all correspondence from Tulsa Achieves will be sent to students’ MyTCC email
accounts.

The signing of this agreement indicates that I, the undersigned, have read and understand the
requirements and restrictions associated with participation in the Tulsa Achieves Program at Tulsa
Community College. My signature indicates an agreement to comply with all eligibility
requirements, submission of documents, and participation in activities. Furthermore, my signature
indicates that the information I have provided on this agreement form is accurate and I understand
that the falsification of information is grounds for removal from Tulsa Achieves and, possibly, Tulsa
Community College.
Tulsa Achieves Program benefits will vary based on length of residency while attending high school
as indicated on the high school transcript in the following ratio. Blanks will automatically be
classified as “Attended only 12th grade” and will receive only 25% funding until corrected
