Foundation 1 Gastroenterology, CIC
The department
The Department of Gastroenterology comprises 2 WTE GI
physicians and 1 visiting GI physician, supported by 2 GI SpRs,
2 nurse endoscopists and 2 hepatology specialist nurses. There
are interests in IBD, hepatology, viral hepatitis, endoscopic
therapy, particularly emergency GI bleeding and hepato-biliary
disease. In addition the team provide input to the acute
medicine rota and ongoing inpatient care for acute/general
medicine as well as in patient gastroenterology.
The
department serves a local population of approximately 200,000
and at a subspecialty level up to 320,000.
The type of work to expect and
learning opportunities
All F1 doctors are typically ward based on a rotational working
day and expected to review gastroenterology and general
medical patients both based on the ward and as out flyers on
the wards.
F1 doctors would be expected to cross cover patients of both
the consultants under the supervision of F2’s and SpRs and be
expected to participate in the acute emergency medical care on
the assessment unit and provide night ward cover for other
general medical patients throughout the building.
The
expectations would be that the ability to take a history, examine
a patient and formulate a diagnosis and initiate a management
plan that sequences appropriately the assessments to reach a
definitive diagnosis.
To liaise with ward staff, PALS, patients and relatives to ensure
all are informed of the state of progress.
To maintain appropriate clinical notes.
To participate in ward audit and governance issues.
Specific experience on practical procedure is likely to include
large volume paracentesis, assessment of decompensated liver
disease and assessment of an acutely unwell patient with
inflammatory bowel disease.

Main duties of the placement
To provide support for registrars and consultants in the
management of inpatients with general medical and
gastroenterological problems. Clinical assessment of patients
and initiation of initial management. Contributing to
multidisciplinary care and management of patients including
interactions with patients, their family, friends and carers.
Undertaking specific practical tasks.
Prioritising care of
inpatients, liaising with middle and senior medical staff and
nursing staff and maintaining good medical records. Prioritising
issues to be dealt with by other staff including senior medical
staff. Contributing to discharge planning and ensuring that
appropriate social and medical arrangements are in place for
discharge home or to other institutions and appropriate
arrangements for on-going care.

On call requirements
There is a 1:14 rota for evening shifts and weekends. There is
no night time commitment in this placement.
Employer information
North Cumbria Acute Hospitals NHS Trust was formed in
April2001 and encompasses both the Cumberland Infirmary,
Carlisle, which was the first PFI Hospital in the UK, (comprising
444 beds), and the West Cumberland Hospital, Whitehaven
(which has 425 beds). The Trust provides a wide range of
Clinical support services and most modern investigational
methods are now available on site. The hospitals primarily
provide services for North Cumbrian residents, but they also
offer services to patients in Dumfries and Galloway, the Borders
and parts of Northumberland. Nearly all specialities are
represented; three are sub-regional and several others are
supra-district in nature.
The hospitals provide healthcare for a catchment area of
around 300,000. A wide range of services are provided with
visiting Consultants from Newcastle
