Checklist for Candidate Interview Folders (Please attach this checklist to the front of candidate folders) Candidate Name: ____________________________________ Please prepare a duplicate file for each candidate referred for interview, and include in each file the following items, along with this checklist: Copy of Approved Position Authorization Copy of Approved Position Announcement (see Academic Openings) Application Letter (include original application letter in one file) CV (include original CV in one file) Copies of all transcripts requested in Position Announcement Letters of Recommendation (3), if required by Hiring Unit, (include original letters in one file) Documentation of all telephone reference calls regarding this candidate according to the following guidelines: Search committee must complete a minimum of five (5) telephone reference calls for each candidate who is being considered for an on-campus interview, as follows: Call to candidate to get acquainted and to ask if search committee may call people who were not explicitly listed by candidates as references; Call to each person listed as a reference (minimum of 3); Call to one person not listed by candidate as reference, unless candidate has denied permission for such calls – if a search committee wishes to continue consideration of a candidate who has denied permission for off-list reference calling, all provided references should be called; Other phone calls as deemed necessary to gather complete information. Routing instructions: 1. Department Head approves the completed web applicant summary, which identifies candidates for interview. The dean is notified by an automated e-mail that the web applicant summary needs dean’s approval. A complete copy of candidate file is delivered to the Dean’s Office. 2. Prior to the development of the EOR and draft appointment letter, forward the Background Check Form to Human Resources. Upon review and approval of the EOR and draft appointment letter, electronically submit EOR and draft appointment letter to the Office of the Provost and forward the selected candidate’s original folder to the Office of the Provost. (If candidate is ultimately hired, the file forwarded to the Office of the Provost becomes the official personnel file.) Upon notice of clearance of background check by HR, appointment letters will be mailed once approval of the EOR is granted. 3.
