2015 ATTACHMENT E HAZARDOUS AGENTS Hazardous agents include radioisotopes, DNA/RNA vectors, infectious agents, transformed cell lines, primary human tumors and hazardous chemicals such as carcinogens and neurotoxins. The use of hazardous agents in animals requires approval from both the UCLA and the University Safety Office. It is the investigator’s responsibility to ensure that the safety office has approved the use of the hazardous agent. What hazardous agent (s) will be used? Is there a Material Safety Data Sheet (MSDS) for the hazardous agent? NO YES If yes, please ensure that one copy is available in your lab, and another copy is provided to the Director of the Animal Facility. Have all personnel in your laboratory been advised of the potential hazards of these agents and provided with the appropriate training? NO YES What dose of agent will be administered to each animal? What is the route of administration? Is the parent compound or any active metabolite excreted in urine and/or feces? What is the duration of excretion? When this agent is used in vivo are there any risks to other animals or to people handling the animals?
