Orientation Process for North Carolina Baptist Hospital
Students/Externs/Interns*
Human Resources is in the process of standardizing its policy and orientation
process for students, externs and interns, to meet Joint Commission
requirements.
Information required for all students/externs/interns prior to start
date:













Full name
Social Security Number (may be sent encrypted or student may call
Pam Morris with the number)
Contact telephone number for the student/extern/intern
Department Number where they will be working
Department Name where they will be working
In-house manager and contact telephone number
Start date of rotation(s)
End date of rotation(s)
Name of college/university
Will they need computer access (specify which systems)
Immunization records including current flu vaccine (see below)
Criminal background check (new requirement starting 1/1/2011)
12 panel drug screen (new requirement starting 1/1/2011)
Process for students:
1. Department (Pharmacy Staff Development) requests required
paperwork and CD or website link for Alternate Orientation from
Pam Morris
2. Student completes Alternate Orientation program and paperwork in
the department. Time required: between one and 1 ½ hours
3. Department (Pharmacy Staff Development) returns paperwork and
CD to Pam Morris, 4th floor, Piedmont Plaza I
4. Student sends the following documentation to Pam Morris for
review by Employee Health Services. Documents must be received
in HR prior to start date





Copies of immunization records, including measles (2), mumps
(2), rubella, chicken pox (proof of vaccine or titer) and tetanus
Proof of seasonal/H1N1 flu vaccination
TB skin test results - must not be older than one (1) year
Criminal background check
12 panel drug screen
5. Student purchases parking pass/badge in Risk Management/
Medical Center Identification, located on the third floor, Meads Hall.
Hours are 8:30 am – 12:30 pm and 2:30 – 4 pm, Monday – Friday
 Fee is $10, payable by check or money order (nonrefundable) made out to Emergency Communications
 A student ID is required (school of pharmacy ID)
 Note: Badges will only be issued if Risk Management/Medical
Center Identification receives prior approval from Pam Morris.
University/College of Pharmacy affiliated students will have the
title “PharmD Candidate” on their WFBMC picture ID badge.
6. Department notifies Pam Morris if instructors are required to be on
site. Badging information will be provided to Risk Management/
Medical Center Identification. Instructors must purchase badges
following the procedure outlined above.
Parking
All students/externs/interns are permitted to park in on the rooftop level of
the Hawthorne Road employee parking deck. Parking passes ($10 fee) are
valid until the end date indicated by the department (end date of last
rotation). If students attend clinical rotations at WFBH longer than one year,
an extension request may be granted upon written request to Human
Resources at no additional charge. Parking passes/badges may be
purchased in Risk Management/Medical Center Badge, located on the third
floor, Meads Hall from 8:30 AM – 12:30 PM or 2:30 PM – 4:00 PM, Monday –
Friday. The students will need to bring their student ID along with the
required non-refundable check (personal check, certified bank check or
money order).
