(Hereinafter referred to as “Official”) hereby agree as follows:
1. It is the business purpose of CCOA to contract with various civic, governmental, and educational institutions to provide officials to officiate various
sports and to contract with individual officials to provide these services.
2. I understand that contracting with CCOA does not guarantee me any specific Number of games.
3. It is specifically understood, and is the express intention of the parties, that the individual Official shall be that of an independent contractor and not
a joint venturer, partner, or employee of CCOA in all dealings and relationships arising out of this agreement. That official shall do the work contemplated
hereunder according to his/her own initiative in accordance with the terms and conditions of this agreement: that any control or right of control by the
CCOA shall be solely over the results to be accomplished hereunder and not to the method of doing the work, except as agreed upon by the parties
to this agreement, and subsequent addendum to said agreement
4. The Official shall use his/her best efforts to officiate all sporting events, which he/she agrees to accept with professionalism, fairness, and with
knowledgeable application of the rules of that sport. The Official acknowledges that the safety of the participants, coaches, and spectators is the
Officials paramount concern.
5. The contract may be canceled, if renewal fees are not paid within 30 days after January 1 of the New Year or if a national background check is not
submitted, at any time by mutual consent of both parties.
6. For a period of three years after the resignation, termination or completion of the officials service to CCOA, the official shall not solicit officiating
business from any school, business, church or other organization for which CCOA provided services during the period of time that the official
provided services to CCOA. In the event of breach of the agreement, the damages shall be liquidated in the immediate amount of $25,000. CCOA
shall retain all other rights in addition to the liquidated damages as stated herein, at equity and law for breach of the agreement. In the event of the
breach of the agreement, the prevailing party shall be entitled to recovery of attorney fees and costs associated with any legal proceeding to enforce
the same.

Note: The Official agrees to pay insurance fees and registration fees into CCOA and further agrees an assessment of 10% of sports
fees tendered.
