completion of this program qualifies for a maximum of __ contact hours. All CE evaluation forms are on-line at www.healthsystemce.org. A valid email address must be provided to MSHP so that
you can sign on to “HealthSystemCE”. If you do not remember your password, click on forgot password and you will receive an email of how to reset your password. A CE code in needed to
verify your attendance at the meeting. The CE code is case sensitive. You will have 60 days from the date of the program to process your CE statement of credit – due date November
19, 2013.
