The New York State Council of Health-system Pharmacists is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing education. Successful
completion of this program qualifies for a maximum of 11 contact hours. All CE evaluation forms are on-line at www.healthsystemce.org with the exception of one presentation: “Impact of
Hyponatremia: Role of the Pharmacist Improving Care”; accredited by University of North Carolina Eshelman School of Pharmacy (see insert in meeting folder). A valid email address must be
provided to MSHP so that you can sign on to “HealthSystemCE”. If you do not remember your password, click on forgot password and you will receive an email of how to reset your password. A
CE code in needed to verify your attendance at the meeting. The CE code is case sensitive. You will have 60 days from the date of the program to process your CE statement of credit – due date November
20, 2012.
