BOOK PURCHASE INSTRUCTIONS
Getting the Surplus Book Request Form

Go to our website, http://www.bham.wednet.edu/departments/purchasing/purchasing.htm
Click on Surplus Books menu.
Click on the Surplus Book Request Form.
From the File Menu, click on Save to save the file to your computer to create the request
form. Save on your desktop or other location you will be able to find the file once it is
completed.

Open the file on your computer you saved in the steps above, Getting the Surplus Book
Request Form.
Go to the alphabetical listing of books on our website
http://www.bham.wednet.edu/departments/purchasing/surplus/index.htm under "Surplus
Book Lists" to select books.
Complete your request electronically in the yellow areas of the form (spreadsheet) provided.
Use the "Tab" key to move through the fields on the form. NOTE: Please be sure to enter
the Box Number for the book(s) you are requesting (supplied on the Surplus Books List).
Print a copy for your records.
Submitting Your Surplus Book Request

Your request may be submitted as follows:
 Electronically as an attachment via e-mail
E-mail the form to julie.rawls@bellinghamschools.org
You will receive a confirmation e-mail within 2 business days of receipt of your
request.
 Faxed to the Purchasing Department at (360) 676-2797, Attn: Julie Rawls
 Mailed or hand delivered to the address listed below.

9. You will be notified when books are available to be picked up via e-mail. Books may be
picked up at the following location:

10. Payment for the books requested is required at the time of pick-up. Total amount due will
show on the bottom of the Request Form.
