Appendix 1 Protocol for arranging proctors for Proctored Final Exams & Procedures for conducting proctored exam Extension Program Coordinators remind students that final exams must be taken in-person at an authorized proctor location during the Learning Preparedness Assessment (LPA) call. o Local students can arrange to take the final exam at the School of Public Health or UC Berkeley Extension. o Overseas students can take the exam at the U.S. embassy under the supervision of a consular officer. o Students should locate and secure a nearby testing center by the end of the second week of the course. Student provides proctor with the Proctor Application and asks the proctor to return the completed application to the Extension Program Coordinator. o Proctor returns application to Extension Program Coordinator via secure fax or as a scanned document. o Upon receipt of completed application, Extension Program Coordinator verifies proctor’s location and email address. o Extension Program Coordinator advises student of approval/denial of proctor. If approved, student is responsible for scheduling exam If denied, student must research alternate proctor and receive approval. Extension Program Coordinator creates and updates proctor tracking spreadsheet for each course. Information on spreadsheet includes: o Course name / number o Student name, phone # and email address o Proctor name, address, phone # and email address o Date of telephone and email confirmations o Approval date o Date exam sent to proctor o Date completed exam received o Date delivered to instructor 24 hours prior to exam date the Extension Program Coordinator sends the exam to the proctor along with the Rules and Regulations document. o Extension Program Coordinator confirms receipt of exam. o Upon completion, the exam is sent to the Extension Program Coordinator via secure fax or scanned email (refer to Rules document). o Extension Program Coordinator delivers exam to instructor to grade. If there is a charge for examination supervision, the student is responsible for the fee. Under no circumstances can a family member or tutor serve as proctor, nor can the exam be sent to a home address. Students are expected to bring a writing implement and a photo ID to the testing site. Proctor is required to verify student’s photo ID and sign cover page of exam. Proctor must agree to monitor the student during the exam to verify that student is not assisted by anyone. Student should not leave the examination (with the exception of an emergency) until it’s completed.
